While operating a loader, EE was cleaning the road with the bucket flat going downhill. The loader dug into the floor causing the loader to stop abruptly. Employee hit EE's head on the front window of the loader.

Mechanic was rotating vee belt sheave checking for operational clearance between sheave and gear box bushing mounting bolts. Head of bolt had a sharp edge, as mechanic was rotating sheave sharp corner of bolt came in contact with mechanic's finger causing a laceration on the end of middle finger on left hand.

We had a breaker trip on our conveyor belt and had to check the motor wiring. EE was removing the putty tape and electrical tape form the wire lugs. EE was using a knife to cut the lugs free and slipped off cutting into EE's left hand EE was holding the cord with. EE was using a safety knife and had leather gloves on.

Employee was changing a screen, went back to the truck to get more bolts and washers. He found an empty bucket in the old service truck and started to pull it out from the shelf. The bucket must have been stuck, so he pulled on it some more until it came loose and hit him in the lip. It was dark outside and he is not aware of how and why it happened.
