EE WAS MOVING A GRIZZLY BAR. AS HE LIFTED THE BAR HE STRAINED HIS LOWER BACK.

EMPLOYEE WAS SEATED ON A CHAIN IN THE CRUSHER IN THE CRUSHER CONTROL ROOM. THE CHAIR BROKE AT THE BASE, HE FELL, HITTING HIS HEAD ON THE WALL OF THE OFFICE. HE SPRAINED HIS NECK, AND RECEIVED A BRUISE TO HIS HEAD.
EE CLAIMS THAT HE STRAINED SHOULDER AND BACK WHILE REPAIRING A BELT. HOWEVER, HE DID NOT REPORT ANYTHING UNTIL THE 11TH OF SEPTEMBER. HE STATES THAT AS HE WAS STEPPING ONTO THE GROUND, HE LOST HIS BALANCE, GRABBED A RAIL TO REGAIN BALANCE AND STRAINED HIS SHOULDER.
STRAINED LIGAMENTS IN LEFT KNEE, WHEN HE STEPPED ON SOME ROCKS HE WAS REMOVING FROM THE CATWALKS.
EMPLOYEE WAS LIFTING A HEAVY METAL GRIZZLY BAR THAT HAD FALLEN INTO THE CONE (THE CRUSHER WAS STOPPED). EMPLOYEE STRAINED HIS BACK.
EE WAS CHANGING THE TIRE ON A SKID STEER. HE WAS PLACING THE TIRE ON THE STUDS, IT SLIPPED OFF THE STUDS AND SMASHED HIS LEFT PINKY FINGER.
