While cutting a zip tie off of a hydraulic hose on the cone, EE was cutting overhead and on a downward fashion with a pocket knife and cut self between the thumb and forefinger in the thumb web. Requiring 7 stitches.

Working in dust cover on jaw, looking at die, hard hat knocked some dirt loose into his face and eyes. Was wearing safety glasses but dirt still went in eyes. did not flush his eyes, went all day with irritation and rubbing them. After 14 hours he went to clinic and right eye was scratched with dirt embedded in eye.

PERSON WAS GRINDING A STEEL PLATE WHEN THE GRINDING DISK FRACTURED & PIECES OF THE DISK STRUCK HIM IN THE ABDOMEN AREA. THE FRAGMENTED PIECES PENETRATED THE ABDOMEN CAUSING THE TRAUMA.
