Machine had received a major repair, personnel was checking machine for leaks, or excessive vibration. While checking machine for a possible vibration issue I placed my hand on the top of a motor fan guard, the guard had a damaged area allowing my finger to come in contact with the motor fan.

Miner was helping to change cutting edges on a loader. As cutting edge bolts were being removed, EE reflexively reached to catch a bolt that had spun free and received lacerations to hand from spinning bolt head.
Miner was working when EE became disoriented for several minutes. EE was taken to medical clinic and diagnosis of EE's condition is undetermined. EE's doctor has referred EE to a neurologist for further tests to determine underlying medical condition.

While driving a haul truck operator went over a rut in the road and jarred back.
Miner was cleaning the floor with a water hose when they slipped and fell, striking their left elbow, resulting in a laceration requiring stitches.
While changing a bearing, Miner hit bearing race with a hammer and a piece of metal broke off, cut through EE's glove and lacerated the middle finger of right hand, requiring stitches.

Injured was thawing out a water valve. Steam built up in the valve and when it released the steam went under the safety glasses and irritated the injured's eyes, resulting in a burn to left cornea. Injured was treated by medical provider and returned to work.

IW was replacing bolts on a cyclone dewatering screen discharge lip. Employee was removing the bolts with a pry bar when the bar slipped. IW's hand slipped and hit the discharge lip cutting their thumb. They received 3 stitches.
Employee was walking between 2 cables at the conveyor line at the press. Left leg made contact with the cable fastener area & the frayed cable end ripped their pants & made a 3" cut on their leg. Employee received 8 stitches.
