INDIVIDUAL WAS MIXING MUD IN A SMALL HOPPER WHEN THE DRILLER ACCIDENTLY STEPPED ON THE FOOT THROTTLE OF THE RIG CAUSING EXCESSIVE PRESSSURE TO THE HOPPER AND IT FLEW OUT OF CONTROL AND STRUCK IDNIVIDUAL IN THE EYE.
