Operator was moving screens by hand into loader bucket to relocate. Screen slipped and his fingers became caught between bucket and screen. Gloves caught on jagged edge of screen material and little and middle finger of left hand were cut, requiring 9 stitches.

Changing counter balance in cone using cutting torch, hammering on counter balance, a piece of metal flew into his eye.

Operator failed to properly park rock truck and did not engage park brake before exiting vehicle. He fell while attempting to urinate from top platform of truck.
