Miner slipped and fell on loose gravel on sloped 3' berm which encloses diesel tank containment area. Left ankle was dislocated and small bone broken.

Miner was shoveling under conveyor. He threw a shovel full of dirt and experienced pain in the lower back. No mining equipment was involved. The miner was trained in these duties and had been observed to be proficient in this task. All protective equipment was in place at the incident location.

EE ""claims"" that he hit a rock with the bucket while operating a loader, somehow jarring his neck.
EE was climbing down a ladder while holding objects. He lost his footing and fell with sufficient force and lacerated his left pinky finger, severing the artery and damaging the nerve.
While grinding a j-bolt during a screen cloth change, a piece of metal pierced the operator's right eye, resulting in pain and temporary vision impairment. The operator claims to have been wearing safety glasses at the time.
The operator lost control of an electric handheld grinder while performing his duties, bringing the grinding disk into contact with his left index finger, resulting in a laceration requiring three stitches.

WE WERE USING A PROPANE WEED BURNER TO THAW OUT A BELT. IT WAS SITTING ON A CLUMP OF DIRT. HE STOOD IN FRONT OF IT AND CAUGHT HIS PANTS ON FIRE SUFFERING 3RD DEGREE BURN ON HIS RIGHT CALF.
