TWO QUARRYMEN HAD ATTACHED 6\ STAGING SLINGS TO DERRICK. AS SLINGS TIGHTENED THEY CAUGHT ON A LADDER LAYING ON THE STAGING CAUSING THE LADDER TO FLIP OVER THE STAGING RAILING AND STRIKE THE VB VICTIM IN THE MOUTH.
