THE EE HAD CUT THE TRACK BOLTS OUT OF THE FISH PLATES ON A 85 PD TRUCK RAIL. HE THEN TOOK A HAMMER AND STRUCK THE FISH PLATES TO LOOSEN THEM. THE RAIL WAS APPARTENTLY ON A BIND BECAUSE WHEN TH E PLATES BROKE LOOSE THE RACK SPRUNG DOWN AND STRUCK THE EE ON THE TOP OF THE RIGHT FOOT.

IN THE PROCESS OF DOING THIS THE INJURED EE'S LITTLE FINTER ON HIS LEFT HAND WAS CAUGHT BETWEEN THE DRAW BAR AND THE LOWBOY FRAME. RESULTING IN A LACERATION REQURING SUTURES. NO LOST WORK DAYS-MEDICAL TREATMENT ONLY.
A ROOF FALL OCCURED ON THE EAST MAINS TRACK AT 147 BLOCK AND EXTENDED INTO THE CROSSCUTS. A ROOF BOLTER ON THE WIRE SIDE CROSSCUT WAS COVERED UP. THE FALL WAS 8 FT TO 10 FT HIGH, 50 FT LONG AN D 18 FT WIDE THE AREA HAD BEEN BOLTED WITH 6 ST FEET RESIN BOLTS NO INJURIES OCCURRED.

A ROOF FALL OCCURRED IN THE EAST MAINS INTAKE ESCAPEWAY AT #7 BLOCK IN AN INTERSECTION #3 ENTRY. THE FALL WAS 8' HIGH, 25' LONG AND 18' WIDE. THE AREA WAS BOLTED WITH 6' RESIN BOLTS AND ADDITI ONAL TIMBERS HAD BEEN INSTALLED.
A ROOF FALL OCCRED ONT EH 10-D SECTION IN THE #1 ENTYR AT #4 BLOCK NEAR THE INTERSECTION (BOLT ENTER). THE FALL WAS 6' HIGH, 18'W AND 20'L. THE AREA WAS BOLTED WITH A 6' RESIN BOLTS. NO INJ URIES OCCURED.
EE WAS HELPING SET A CMI SCREEN DOWN INTO A CMI. WHEN ONE OF THE EES HELPING LOST HIS GRIP ON THE SCREEN AND DROPPED IT, PINCHING EE'S INDEX FINGER ON HIS LEFT HAND CAUSING A FRACTURE TO THE T OP OF THE FINGER AND A LACERATION.
THE MEDICAL DOCTOR HAS DIAGONISED THE EE WITH HAVING CARPAL TUNNEL SYNDROME IN HIS LEFT & RITHT WRIST. HE HAD SURGERY ON 8-28-07 & BEGAN MISSING WORK. HE IS TO HAVE SURGERY ON HIS LEFT WRIST I N THE NEAR FUTURE.
WHILE UNCOUPLING A SUPPLY CAR, THE ROPE FOR THE UNCOUPLER BROKE. THE EE THEN USED A JACK BAR FROM THE LOCOMOTIVE TO TRY TO RELEASE THE UNCOUPLER. THE JACK BAR SLIPPED OFF THE UNCOUPLER AND EE MASHED HIS FINGER BETWEEN THE JACK BAR & EDGE OF THE SUPPLY CAR.
IN THE 10-D BELT ENTRY INBY 11 BLOCK, A 8" I BEAM WAS BEING UNLOADED FROM A PETTI MULE. ONE END OF THE I BEAM WAS SECURED TO A CHAIN AND THE MULE WAS BACKING OUT, THE I BEAM CAME OFF AND STRUC K EE ONT HE LEFT LEG.
A ROOF FALL OCCURED ON THE 10-D SECTION IN THE 1 ENTRY IN THE INTERSECTION OF 12 BLOCK. THE FALL WAS APPROX. 16' HIGH, 17' WIDE AND 39' IN LENGTH. THE AREA WAS BOLTED WITH 6' RESIN BOLTS, ROPE TRUSSES, SUPER POST, 4" BOX BEAMS AND CRIBS NO INJURIES OCCURRED.
ON HE 10-D LONGWALL SECTION IN #3 ENTRY EMPLOYEE WS TRYING TO MOVE LONGWALL PARTS SO THEY COULD BE LOADED WITH THE FORKS OF THE MULE EMPLOYEE JERKED ON PART FELT PAIN IN THE GROIN AREA, CAUSI NG A HERNIA THAT WITH REQUIRE SURGERY.
ON THE 10-D LONGWALL SECTION EMP WAS OPERATING THE OUTBY SHEAR DRUM. AT #16 SHIELD THE DRUM THREW A PIECE OF SLATE ROCK OVER THE PANLINE AND STRUCK THE EMP IN THE RIGHT FOOT CAUSING A FRACTURE TO THE TIP OF THE BIG TOE. A FRACTURE TO THE SECOND TOE AND TWO FRACTURES TO THE THIRD TOE ALSO AN OPEN WOUND TO THE THIRD TOE.
ON THE 10B SECTION AT #11 BLOCK, XCUT #2-3M EMP WAS HELPING MOVE SUPORT LEGS THRU A MANDOOR. WHEN SLIDING A LEG ACROSS THE TOP OF THE BUNDLE 5 OF THE LEGS FELL STRIKING EMP IN LEFT LEG CAUSING A FRACTURE OT TIBIA AN FIBULA BONES OF LEFT LEG
A ROOF FALL OCCURRED ONT HE 10-0 LONGWALL SECTION IN 3 ENTRY AT 14 BLOCKS THE FALL WAS 16' LONG, 16' WIDE AND 8' HIGH. THE AREA WAS BOLTED WITH 6' RESIN BOLTS. NO INJURIES OCCURRED.

EE WAS WALKING ALONG THE LONGWALL FACE AT APPROX #10 SHIELD. HE STEPPED ON A WET RAM JACK, SLIPPED AND FELL DOWN. HE ATTEMPTED TO CATCH HIMSELF WITH HIS RT HAND. THIS CAUSED A HAIRLINE FRACTUR E TO THE RT WRIST.
THE MEDICAL DOCTOR HAS DIAGNOSED EMPLOYEE WITH HAVING CARPAL TUNNEL SYNDROME IN BOTH HIS LEFT AND RIGHT WRIST. HE HAD SURGERY ON 6-13-97 AND BEGAN MISSING WORK. HE IS TO HAVE SURGERY ON HIS LE FT WRIST IN THE NEAR FUTURE.

EE WAS REMOVING A COUPLER FROM A DWI-DAG TRUSS ROOF BOLT WHEN THE COUPLER AND ROOF BOLT FELL CATCHING HIS MIDDLE FINGER ON HIS RIGHT HAND BETWEEN THE 1ST KNUCKLE AND THE NAILBED BETWEEN THE CO UPLER AND THE TOP OF THE CRUSHER FRAME.
A ROOF FALL OCCURRED ON THE 10-0 SECTION BELT LINE 1 ENTRY BETWEEN. 19 AND 20 BLK IT WAS 60' LONG 12' HIGH AND 18' WIDE THE AREA HAD BEEN BOLTED WITH 6' RESIN BOLTS AND ROPE TRUSS BOLTS NO INJ URIES OCCURED.
ON HE 10-0 LONGWALL SECTION EE WAS REMOVING TRACK RAILS WITH 2 OTHER EE INJURED EE ATTEMPTED TO MOVE A PULL ROPE BEING USED TO MOVE TRACK RAILS. AT THE SAME TIME THE OTHER TWO EE WERE SLOWING A TRACK RAIL, THE RAIL STRUCK HIM IN HIS RIGHT ANKLE CAUSING HIM TO FALL, INJURING HIS RIGHT ANKLE.

ON THE 10-0 LONGWALL FACE, EMP WAS STANDING ON #38 SHIELD ADVANCING #39 SHIEL WHEN THE RETURN LINE ON #38 SHIELD CAME OUT OF THE SUPPLY BLOCK SPARAYING EMP WITH WATER. WHEN THE WATER HIT HIM H E JUMPED ON #39 SHIELD THAT WAS ADVANCING CATCHING HIS RIGHT FOOT BETWEEN THE DRAW HEAD AND THE TOE OF THE SHIELD CAUSING A FRACTURE TO THE RIGHT FOOT.
A ROOF FALL WAS FOUND IN WEST MAINS TRACK AT 280 CROSSCUT, IN A 3-WAY INTERSECTION. IT WAS APPROX 18'LX40'WX6'H. NO INJURY OCCURED.

EE HAS A LENGTHY HISTORY OF INJURIES, HE IS NOW COMPLAINING OF PAIN FOR VARIOUS PROBLEMS. ADDITIONAL DAYS WILL BE FORWARDED WHEN EE RETURNS/TERMINATED ETC.(SEE ENCLOSED HISTORY).
EMPLOYEE FIRST REPORTED NECK PAIN THEN BACK PAIN.
THE MEDICAL DR. HAS DIAGNOSED EE WITH HAVING CARPAL TUNNEL SYNDROME IN BOTHE HIS LEFT AND RIGHT WRISTS. HE HAD SURGERY DONE ON HIS LEFT WRIST ON 6-2-97 AND IS TO HAVE HIS RIGHT WRISTE OPERATED ON IN A FEW WEEKS.
INJURED AND 3 OTHERS WERE LIFTING ROLL OF BELT ONTO THE BELT LINE WHEN HE FELT HIS BACK POP.
ON THE 10-D SECTION BELT LINE EE WAS HANGING MONARAIL & EXPERIENCED PAIN IN THE LOWER STOMACH & GROIN. HE CONTINUED TO WORK UNTIL 1/8/97 WHEN HE WAS DIAGNOSED TO HAVE A DOUBLE HERNIA, REQUIRIN G SURGERY.

A ROOF FALL WAS FOUND IN THE OLD 7-D SECTION 1 & #3 ENTER BETWEEN #2 & #3 BLOCK. THE FALL WAS 80' LONG 17' WIDE AND 6' TO 8' HIGH. 6' RESIN BOLTS WERE USED IN THE AREA.
