Miner was attempting to unclog a pipe with another pipe using compressed air. The smaller pipe, that was inside the larger clogged pipe, slipped due to moisture, hitting the top of the miner's right hand with the flat piece of metal, cutting the top of EE's hand open.
Miner was cleaning off a truck and there was a patch of snow on the ground near them. They slipped, but did not fall. When they went to catch themselves, they pulled a muscle in lower back.

EE was standing behind a log washer lifting a small screen holding back small rocks. As EE was prying it, a rock flew up and struck EE in the mouth. This caused a left molar to completely dislodge.

Employee states that ee was driving a quarry truck along a haul route through the quarry when ee hit a pot-hole and the truck bounced. EE felt a pinch in lower right side of back immediately, but that the pain slowly grew over the course of the next week until ee reported the injury on 09/28/2018.

EMPLOYEE WAS CHANGING OIL ON CAT MOTOR. HE HAD TO GET MORE OIL & AS HE TURNED AROUND HIS RIGHT FOOT HIT THE LITTLE SLOPE WHERE THERE IS A 2" PIPE ATTATCHED TO WHICH IS A FLAT PIECE OF STEEL HE SLIPPED & HIS LOWER LEG HIT THIS PIECE OF STEEL AS HE FELL. HE HAS A FRACTURED FIBULA.

THE LARGE TORCH FELL TO THE GROUND AND THEFLAME WENT ACROSS HIS LEFT HAND, ON THE KNUCKLE SIDE. EMPLOYEE'S LEFT HAND WAS BURNED STRAIGHT ACROSS HIS LOWER KNUCKLES.
