Miner was holding a long piece of rebar, torching it, when the bottom half of the rebar fell once they cut through and landed, straight up and down, onto their right shin. Causing a 3.5'' gash in their leg, down to the bone.
Two miners were in the fab shop making a repair on a WA700 Hydraulic Tank. EE began to weld with EE's left arm inside the cylinder, while other EE held a flashlight above with right hand. The first weld (about 1 inch) did not cause any issue, EE then began to weld again, and immediately the cylinder ignited causing a full flame blast up EE's arm. It also engulfed other EE's hand.

At approximately 11:30am, EE (newer hire) was in a scissor lift lifting and bolting sheet steel. While in the lift, the piece of sheet steel shifted and fell, jerking EE in the process. When the falling steel jerked EE, it caused a slight displacement in EE's back and strain.
Our employee was exiting a loader that they were cleaning and when exiting down the ladder, they missed the last step and fell onto their left ankle/foot, twisting it.

While working under the bucket of a wheel loader noticed that one of the braces against the cylinder of the bucket was unstable. While resolving this, one of the braces came loose and struck EE in the left eyebrow and shoulder, causing EE to fall back into a bolt rack. This caused bruising on EE's shoulder and lower right back with some bleeding from the left eyebrow.

Employee was sorting 1/2" x 4' x 4' steel sheets stacked vertically against a handrail. Employee leaned three sheets against self to access the fourth sheet in the stack when all four sheets tipped over onto legs. Incident resulted in a fracture to the right lower leg.
