Miner was holding a long piece of rebar, torching it, when the bottom half of the rebar fell once they cut through and landed, straight up and down, onto their right shin. Causing a 3.5'' gash in their leg, down to the bone.

Two miners were in the fab shop making a repair on a WA700 Hydraulic Tank. EE began to weld with EE's left arm inside the cylinder, while other EE held a flashlight above with right hand. The first weld (about 1 inch) did not cause any issue, EE then began to weld again, and immediately the cylinder ignited causing a full flame blast up EE's arm. It also engulfed other EE's hand.

The second weld immediately caused the cylinder to ignite causing a full flame blast up other EE's arm. It also engulfed EE's hand.

At approximately 11:30am, EE (newer hire) was in a scissor lift lifting and bolting sheet steel. While in the lift, the piece of sheet steel shifted and fell, jerking EE in the process. When the falling steel jerked EE, it caused a slight displacement in EE's back and strain.

Our employee was exiting a loader that they were cleaning and when exiting down the ladder, they missed the last step and fell onto their left ankle/foot, twisting it.

While working under the bucket of a wheel loader noticed that one of the braces against the cylinder of the bucket was unstable. While resolving this, one of the braces came loose and struck EE in the left eyebrow and shoulder, causing EE to fall back into a bolt rack. This caused bruising on EE's shoulder and lower right back with some bleeding from the left eyebrow.

Employee was sorting 1/2" x 4' x 4' steel sheets stacked vertically against a handrail. Employee leaned three sheets against self to access the fourth sheet in the stack when all four sheets tipped over onto legs. Incident resulted in a fracture to the right lower leg.

Employee was dislodging a jam in a rock feeder when the pry bar being used shifted, causing hand to strike the surge tunnel wall.

Employee lifted a section of water pipe and felt a strain in lower right abdomen. Once symptoms didn't resolve, employee sought medical treatment and diagnosed on 11/11/14 as hernia.

Employee was pushing on a tail pulley and felt pain in upper right thigh. Inguinal Hernia.

Employee was removing the front cover from a haul truck and fell hitting his left wrist on the ground causing a wrist fracture.

Employee was assiting to connect crane cable to conveyor structure on ground, when he went between opening in conveyor, he slipped and his shin struck the metal cross members.

Employee was cutting old conveyor belts to be used as cleanup mats and he was using a utility knife to enlarge holes and the knife slipped from his hand and cut his left palm.

Employee was cutting hole in coveyor belt in order to put chain through in order to pull belt off conveyor. Utility knife was hard to pull out and when employee pulled, it struck employee in the inner left thigh. Employee recieved 2 stiches.

EE CLAIMS HE WAS PUTTING A NEW BELT ON A MOTOR DRIVE & HIS RT. PINKY WAS PINCHED BETWEEN THE BELT & DRIVE.

EE CLAIMS THAT AS HE WAS OPERATING AN END-LOADER, A PIECE OF STONE CAME OFF THE BUCKET WHILE LOADING A QUARRY TRUCK AND HIT HIS CAB, PUSHING THE CAB INTO HIS RIGHT KNEE.

10-25-95 WAS THE FIRST MORNING OF FROST. EMPLOYEE WAS STANDING ON CONVEYOR, LOST BALANCE AND JUMPED OF FELL TO THE GROUND, LANDING ON FEET. BROKE HEELS IN BOTH FEET. SUBSEQUENTLY DROPPED TO TH E GROUND AND FRACTURED RIGHT WRIST.

EE PARKED FORLIFT IN GEAR ON INCLINE. FORLIFT ROLLED. EE MOVED OUT OF THE WAY AND STUMBLED AGAINST A ROCK, CRACKING 3 RIBS AND BRUISING HIS SPLLEEN.

EE WAS SCRAPING CLAY OFF FEEDER PAN AS HE WAS LEAVING AREA A ROCK FELL OFF LEDGE HITTING HIM ON RIGHT BUTTOCK.

ICEY & SNOWY PULLED HAMSTRING WITH MICRO TEARS

EMPLOYEE WAS CARRYING A METAL PLATE DOWN TO THE STONE CRUSHER-TURNED AND TWISTED LEFT HIP.
