The miner did not notice that a pick-up truck was driving in reverse and was hit by the side of the truck. The driver of the truck stopped immediately once he notice the miner was in the ground. The truck was subject to inspection by MSHA personnel September 4,2014 and there was no deficiencies.

Supervisor was verifying plant when he took a bad step causing his right knee to go backwards & causing him to fall. This occurred 2/13/07 & he decided to go to a private doctor. The orthopedic doctor after evalutating the MRI decided it was necessary to operate on a ligament in the right knee.

THEY WERE REMOVING A HYDRAULIC CYLINDER TO CHANGE THE SEAL. THE CYLINDER IS HOLD WITH A PIN TO THE BASE OF THE CRUSHER, WHILE REMOVING THE PIN THE CYLINDER PITCH THE FINGER.
EE WAS INSTALLING A LOUDSPEAKER TO HIS VEHICLE WHEN HE WAS DISMOUNTING FROM THE CAR, HE WAS HIT BY ANOTHER CAR BY HIS CO-WORKER. EE FELL TO THE GROUND ROLLED FURTHER DOWN THE ROAD AND THE CARS REAR TIRE RAN OVER HIS RIGHT LEG.
EMPLOYEE WAS STORING THE ACETYLENE CART IN THE SHOP. HE TRIPPED THE CART FELL ON HIS HANDS AND HIS MIDDLE FINGER WAS PINCHED.

THIS EE CLAIMS THAT ON MAY 28TH HE SLIPPED WHILEDRILLING AND HIS BODY WEIGHT FELL ON HIS LEFT SHOULDER. IT WAS NOT UNTIL JUNE 17TH THAT HE REPORTED THE INCIDENT.
WHILE EMPLOYEE WAS DOING HER WORK, TWO INDIVIDUALS VIOLENTLY INTERRUPTED INTO THE OFFICE FOR A ROBBERY. HER FINGERS WERE INJURED AFTER THEY THREW DOWN THE DOOR. SHE ALSO SUFFERED A NERVOUS ATT ACK & SEVERE HEADACHE.
INJURED WORKER WENT TO GET ASLACH AHMMER TO BREAK A ROCK IN THE FEEDER, WHILE THE FEEDER WAS RUNNING. THE FLOOR WAS WET, BECAUSE IT WAS RAINING, AND HE SLIPPED AND FELL IN THE FEEDER AND THE ROCK TO BE REMOVED FELL OVER HIM. THE FEEDER DRAGE HIM AND HE GOTS HIS RIGHT ARM AND SHOULDER BROKEN.
