OTHER EMPLOYEE WAS USING A 3' PIPE ON THE END OF A 3' BREAKER BAR TO TIGHTEN A NUT ON THE JAW CRUSHER. THE PIPE FELL OFF AND STRUCK EE IN THE LEFT EYE AREA.

I NAME WAS HOLDIGN A PC OF STEEL PLATE UP AGAINT THE SIDE OF BIN BY A ROPE AND PULLEY SYSTE M STEEL PLATE BEGAN GO FALL AND HE LET GOT OF THE ROPE AND TOOK A STEP BACK AWAY FORM STEEL WHEN HE STEPED BACK HE STOOD ON A PC. OF ROPE AND AS THE STEEL TOOK PC OF ROPE DOWN TO GROUND THE PC. HE WASS STANDING ON WENT OUT FORM UNDER HI M. CAUSING HIM TO FALL TO GROUND BELOW
