EMPLOYEE ATTEMPTED TO MOVE A CONVEYOR TAIL PULLEY WITH A 2 WHEEL DOLLY, AS HE PULLED BACK THE PULLEY SLIPPED OFF AND HE LOST HIS BALANCE AND FELL BACKWARD ONTO A WOODEN BLOCK.
