On 2/20 at 11:30, employee was operating the doghouse at Avoca Quarry when EE went to clear a cone bridge using a pry bar. While pulling on the pry bar, the rock gave way and EE hit pinky finger on the cage surrounding the crusher inlet, resulting in multiple open fractures of the tip of that finger.

EE was using a screw driver to pry open a bung seal in a 55 gallon drum. The screw driver slipped, which caused his hand to glance across the aluminum seal. He suffered a right hand laceration that required 25 stitches. EE was using the wrong tool to get the job done, resulting in this injury.

Ee was shoveling rocks at the avoca quarry on August 23rd. He woke up the next morning with pain from the left lower back area to the left upper back and said he was having pain, numbness and tingling going down both his legs. He went to the er on his own and was treated for back strain. He informed his supervisor on august 23rd and was sent to oahc to be checked

EE was performing maintenance on a crusher with an impact wrench. The bolt broke on jaw crusher and he caught his right hand between the wrench/I-beam causing a laceration between the thumb/index finger. He went to aohc and received 8 stitches and was bandaged. He was also given a tentanus shot and 2 prescriptions. He returned to work the nexd day.

EE was working on shaker screen in which he had implemented LO/TO procedures. He finished his work and then removed his lock and flipped the braker switch back on. An Arc Flash occurred when braker switch was activated causing 2nd degree burns to his face, lips, chin and neck.

HE WAITED FOR THE BOLTS TO BE CUT OFF BY TORCH THEN WAS USING A 18" PRY BAR TO PRY THE ROLLER OUT OF THE BRACKET. HE WAS PUSHING ON THE PRY BAR WHEN IT SLIPPED AND HE LET GO OF THE BAR AND THE BAR HIT HIM JUST UNDER THE LEFT EYE LID
