While backing from the bank and preparing to load a haul truck, a ~2-ft. rock fell from the loader bucket. The operator did not see the rock, ran over it, and the loader became unstable and overturned on level ground.

Employee was in the process to change out steel silica fines screen. The old screen was hung-up on a mounting bolt. Employee was using a hand shovel & was attempting to brake the old screen away from the bolt. His hand slipped on the shovel handle & his finger was caught between the shovel handle & the screen edge. His finger was cut, requiring sutures to close it at the E.R.
