Employee tripped over a piece of dunnage under structural steel they were covering with plastic concrete blankets and fell to ground at the same level. The right distal radial bone at the wrist was fractured.

New construction, and addition of a metal flat strap piece was in process of installing. Employee had one end of an 8 inch by 20 ft piece of flat strap clamped and attempted to position the piece in place. The clamp came loose, and the flat metal strap struck EE above EE's left eyebrow. Took employee to the local Dr. Six stitches required and released to full duty.

Employee working on the steel structure of the Sizing Tower tightening bolts on the cross member. The employee was working from a scissor lift to impact bolts with an impact, and sustained debris in their right eye.
