WAS ON A ALUMINUM LADDER, WORKING AT THE ASPHALT PLANT, WHEN HE FELT THE LADDER WAS GOING TO FALL AND IN TURN HE GRABBED TO THE SIDE OF PLANT, STRAINING HIS LEFT SHOULDER, AS TO KEEP FROM FALL ING.
THE EMPLOYEE EXPERIENCED LOWER BACK PAIN CAUSED BY OPERATING A RUBBER TIRED FRON TEND LOADER.THIS CONDITION WAS CAUSED BY NORMAL OPERATION OF THE LOADER.INJURY APPEARS TO BE TEMPORARY.
IN THE NORMAL RERFORMANCE OF HIS DUTIES(NAME) WAS INSPECTING THE QUARRY AREA,WHEN HE STEPPED ON AL LARGE ROCK.HIS WEIGHT CAUSED THE ROCK TO SHIFT CAUSING HIM TO LOSE HIS BALANCE.UNABLE TO REG AIN BALANCE,HE FELL AND JUMPED FORWARD APPROXIMATELY 3' LANDING ON SMALLER LOOSE ROCKS,CAUSING HIM TO SPRAIN HIS RIHGT ANKLE AND DISLOCATE HIS LEFT HUMB.
