Employee was unplugging the ref chute on 7th floor. As EE was attempting to dislodge the material with a slate bar, the tip of the bar slipped off and caused EE's prying arm hand to pinch between the bar and the chute door.

While employee was stepping down off a Caterpillar 740 articulating truck to refuel, EE's foot slipped on the last step. EE caught self with right arm and felt pain in right shoulder and the pain also radiated to EE's neck. Note: Employee continued to work until 10/11/2022 when surgery was needed.

Employee unplugged course refuge bypass chute that was blocked by a crib block. After removal, employee was standing on the floor below watching and listening to ensure start-up would be successful. Unbeknownst to the employee, spillage had occurred on the floor above. During start-up, a piece of rock fell and hit the employee on the head.

Employee was preparing to unload a plate press panel from a bed of a truck when a large gust of wind blew three panels over, striking him in the low back and knocking him down.

Employee was using a sledge hammer to beat old ceramic tile out of a ceramic lined box. As he pulled the hammer back and started to swing in a downward motion, he felt a sharp pain in his left shoulder.

Mine Supervisor asked EE to clean up area where he was working to see why water was dripping on them. EE hosed down area and there was a hole. When water broke the hole open, EE was dumped in to the chute and covered up.
Down on knees winching. As EE was getting up his knees gave out.
Injured was loading truck at refuse bin. Remote would not shut gate, so he exited truck to use manual gate control. He hit his head on a piece of metal conduit, knocking his hat off and cutting the top of his head. Required stapes to close the wound.
On the above date, the injured was shoveling coal in the clean coal tunnel when he felt a sharp pain his left hip and lower back. After being evaluated by a doctor it was found that he had a degenerative disc issue and until the date of April, 19th of 2010 he had resumed work until the old injury botherd him enough to seek further medical assistance. And be off from work.
Injured was usaing a torch to cut wheel straps loose from wheel of rail car. While doing this a piece of hot slag went in his ear causing a burn. Sought medical treatment on 3/23/09.

Employee was at the discharge chute of the raw coal screen removing trash from the grizzley when he observed a feeder flight coming across the raw coal screen. He tried to catch the flight to keep it from going into the heavy media vessel. The flight was too heavy and caused a pain in his left elbow.
Employee was leaned over talking to his foreman (who was in a vehicle). When employee straightened up, he stated he felt a sharp pain in his lower back. The employee has not missed any work. After a follow-up visit with his physician, it was recommended that the employee start physicial therapy on Monday 11-10-08.
Lifting swivel from 321 excavator. Umbilicas Hernia, lifting. Light duty for 2 weeks, no lifting.
