"WHIPLASH" NECK INJURY. EE WAS DRIVING WATER TRUCK, HIT BUMP, APPLIED BRAKES HARD, HAD MARKS TO WHERE TRUCK CAME TO A STOP. HE SAYS HE HEARD A "POP" IN HIS NECK AND FELT NECK PAIN. TRANSPORTED BY LOCAL RESCUE SQUAD.

THE SHEAVE THAT THE CABLE RUNS THROUGH ON THE WINCH WAS TWISTED, WHEN HE TRIED TO UNTWIST THE CABLE, THE CABLE RELEASED AND CAUGHT HIS FINGER AND PULLED IT INTO THE SHEAVE.

AT 9:20AM EE WAS HOLDING WRENCH ON 2IN NUT AT JAW CRUSHER HE WAS TIGHTNING THE NUT ON THE BOLT THAT HOLDS CAP ON TOP OF THE CRUSHER BEARINGS EMPLOYEE EE WAS SWINGING A 20LB HAMMER TO HIT THE W RENCH TO TIGHTEN THE NUT ON THE BOLT EE LEFT PINKY WAS MASHED/CUT TO THE BONE WHEN IT GOT PINNED BETWEEN THE WRENCH & THE NUT
THE EMPLOYEE WAS HELPING TO REMOVE AN OVERSIZED ROCK FROM THE JAW CRUSHER CAVITY. AS HE LEANED OVER THE BOX TO STRIKE THE TONGS WITH THE SLEDGE HAMMER THE LEFT SIDE OF THE TONGS PULLED OUT OF THE ROCK CAUSING THE BLOCK TO STRIKE EMPLOYEE ON THE LEFT SIDE OF THE HEAD NEAR HIS TEMPLE.
