Removing a boulder from crusher. The boulder broke and fell on left big toe causing swelling, redness, tingling, & pain.
EE was standing with other employees talking to the supervisor. EE fell on the ice on EE's buttocks & right shoulder causing right shoulder pain & chest pain.

HE SUFFERED A DISLOCATED RIGHT SHOULDER. HIS SHOULDER WAS REPAIRED AT THE HOSPITAL EMERGENCY ROOM UNDER ANESTHESIA.
