Employee was exiting the rock breaker when employee missed a step and fell, ultimately breaking two bones in employee's wrist.

The employee was using a wrench to loosen wedge bolts that hold the bowl liner in the crusher. The employee leveraged the wrench to release the bolt. Immediately, EE felt a sharp pain in EE's left foot as EE's weight transitioned. An x-ray was performed and revealed a fracture of the fifth metatarsal bone in EE's left foot.

After speaking with a customer in the yard, the injured employee climbed back into the loader's operator chair to continue working. From the chair (in a sitting position) the employee reached for the door to pull it shut with right arm. The door was not locked or latched. While pulling the door shut, EE felt a sharp pain in lower back.

ONE WAS HANDING THE TIN TO OTHER WHO WAS IN THE BED OF THE TRUCK & HE WAS HANDING THEM TO INJURED (WHO WAS ON TOP OF THE CAB OF THE TEREX TRUCK). MOST OF THE PIECES WERE 8' IN LENGTH. INJURED HAD TURNED TO PUT ONE PIECE OF THE ROOF. HE TURNED TO GRAB THE NEXT PIECE THAT WAS 12'LONG & RAN INTO IT CUT NOSE
