EE WAS OPERATING THE 801 EXCAVATOR REMOVING CLAY MATERIALS FROM A SPOIL BANK. AN UNKNOWN AMOUNT OF CLAY BROKE FLEE AND STRUCK THE FOREARD OPERATOR GLASS OF THE EVCAVATOR CAUSING IT TO SHATTER. THIS IN TURN IS BELIEVED TO HAVE CAUSED THE OPERATOR TO MAKE A SUDDEN MOVE INSIDE THE CAB AND STRIKE HIS LEFT KNEE AGAINST THE BRACE OF THE MACHINE RESULTING IN THE INJURY OF HIS LEFT KNEE.

EE WAS WALKING ACROSS WALKWAY OF MINER LAUNCH PAD OF HIGHWALL MINER. HE ALLEGED HIS FOOT SLIPPED & HIS KNEE WAS LACERATED WHEN IT STRUCK THE METAL WALKWAY. LACERATION WAS APPROX 1/4 " LONG.

EE WAS STEPPING OFF A SET OF STEPS FROM LAUNCH PAD OF THE HIGHWALL MINER. HE STEPPED IN A HOLE ON PIT FLOOR. HE ALLEGED THAT HE TWISTED AND STRAINED HIS HIP. HE STARTED MISSING WORK DUE TO THIS ACCIDENT ON 5-24-02.

EE WAS PULLING CABLE FOR THE HIGHWALL MINER AS IT WAS TRAMMING TOWARD FACE. THE ADD CARS WERE TRAMMING OFF FRONT OF LAUNCH PAD. THE WHEELS DROPPED DOWN CAUSING THE CARS TO DROP AGAINST PAD. HI S FOOT GOT CAUGHT BETWEEN CAR AND PAD FRACTURINGHIS BIG TOE & FIRST TOE ON HIS RIGHT FOOT.

WINCH LINE BROKE ON DOZER THAT WAS HOLDING ANOTHER DOZER CLEARING BRUSH ON SLOPE. DOZER SLID DOWN HILL TURNING OVER APPROX. 4 TIMES, BLADE TO WINCH. DOZER ROLLED ON 60 DEGREE SLOPE ABOUT 400' . OPERATOR WAS IN BRUSH CAB WITH SEAT BELT ON.
