Employee was on their way to deliver ground support mats to the miners with telehandler. One bundle of mats fell off the forks so the employee stopped to re-load and position the bundle of mats. While doing this the second bundle of mats slid down the forks and pinched the right hand injuring the rt middle and ring finger.

while cutting off mats during rehabilitation work the employee went to throw aside a cut off mat and the edge of the mat lacerated EE's left bicep.

While traveling out from the #2 area of the KT tunnel Loci #10 derailed causing it to come to an abrupt stop. This abrupt stop caused employee to hit RT Knee on the metal seat bracket causing an abrasion. There was no medical treatment received, no lost time, no restrictions, and employee continued to work employee's regular hours.

Miners were progressing through a known fault zone. Mining methodology had been adapted (timber ground support was put in place) but ground conditions remained bad. The injury sustained was not life threatening. The miner incurred a laceration to the left leg while setting up to shotcrete. EE will make a full recovery.

A fire occurred at the #1 shaft station generator concreted location. Investigation found the potential cause of the fire was a ground fall in a timbered area of the drift, causing fallen timber to come in contact with the exhaust of the generator. The fire was of unknown duration.

Support timber collapsed in the #2 shaft below the 10th level, blocking access to the pump hung below the level, used to dewater the shaft. Time of collapse unknown, no one was present to witness.

Individual was guiding a skid steer in order to replace a timber car back on the track. Individual was walking backwards and tripped. Extended an arm to catch himself and landed with their hand on the ground and the weight of their body fractured the left arm approximately 2" above the wrist.

Employee was using a 4 1/2" grinder with a cutting wheel attached to cut PVC pipe. Grinder slipped and hit employee on the right thumb causing about a 1" laceration. Employee was taken to the clinic where he received 2 stitches.

Employee was drilling a hole in a piece of pipe, using an electric drilll mounted on a stand. As he pulled the pipe out, the drill bit caught his glove and pulled his left hand index finger into the hole saw blade.

As EE was using a grinding wheel to remove paint and rust from skip he turned his head to look ahead of his work, a particle flew in the side of his safety glasses and lodged in his eye. It did not bother him until the next morning. It was removed by his dr.

EE WAS BARRING DOWN LOOSE ROCK IN THE RAISE. THE 1ST ROCK KNOCKED OFF HIS HARD HAT AND THE 2ND ROCK HIT HIM ON THE TOP OF HIS HEAD, RESULTING IN A CUT. HE WAS TREATED AT THE CLINIC, AND RETURN ED TO WORK THE NEXT DAY.
