Miner was walking in decline area and slipped. His left foot slid clear and his right foot caught on a rock and buckled under him. Initially the miner felt he was ok and did not feel it necessary to seek medical treatment. On 3/9/2015, he decided he needed medical treatment. He worked from 2/25/15 until 3/9/2015 without any restrictions.

Employee was flipping over a piece of steel, had gloves on, and the metal slipped and pinched his ring finger on the left hand.

While preparing to load drill holes with ANFO miner grabbed a rock with his hands. The rock slipped lacerating two of the miner's fingers on his left hand. The lacerations required stitches.

"Dribble" was observed in the N12 heading as a worker worked in the cross-cut V24. The "dribbling" continued for a period of time estimated at approximately 5 minutes. After that time a section of the roof fell. The material seen on the floor averaged approx. 12"-24", and amounted to an est. 100 Tns. No damage or impairment caused.

On Monday, March 19,2007-Leadman called the surface office about 8am to inform me of a roof fall at the intersection of NE 10 @ U. This area had previously been closed off to access on 2-6-07. This area was not an active mining area. The roof fall was approximately 30' wide and 40' long forming a bowl in the roof roughly 20 feet high. MSHA was immediately called.

Roof fall approx. 1100" down slope from portal causing damage to steel roof supports. Roof was supported by steel sets 24' wide consisting of 12 X 65 horizontal beams and 2 ea 10 X 54 vertical legs, w/4" x 6" wood lagged and cribbed w/ 6" hardwood.
