BURNING SPRINGS MINE: 426 MSHA citations Metal/Non-Metal

Unknown ignition in working area.

Working at the face when a small flame appeared in the muck pile.

Employee was stepping down from the equipment and rolled right ankle.

Miner was walking in decline area and slipped. His left foot slid clear and his right foot caught on a rock and buckled under him. Initially the miner felt he was ok and did not feel it necessary to seek medical treatment. On 3/9/2015, he decided he needed medical treatment. He worked from 2/25/15 until 3/9/2015 without any restrictions.

Employee was flipping over a piece of steel, had gloves on, and the metal slipped and pinched his ring finger on the left hand.

While preparing to load drill holes with ANFO miner grabbed a rock with his hands. The rock slipped lacerating two of the miner's fingers on his left hand. The lacerations required stitches.

Cutting out pin retaining bolt, bolt broke and struck employee in chin causing laceration requiring 4 stitches...no lost time.

An unintentional detonation of an undetected charge of explosive material occurred while the Gradall scaler was removing loose rock from the back and ribs of the SW-2 face heading.

Employee was hand scaling and a rock fell striking his right hand causing a laceration on the bottom part of thumb and fore finger

Employee was installing cab protection guards. He was getting down from upper deck and slipped twisting his right knee and overextending it. The entire machine had a thin layer of ice on it, including the sheet metal on it.

"Dribble" was observed in the N12 heading as a worker worked in the cross-cut V24. The "dribbling" continued for a period of time estimated at approximately 5 minutes. After that time a section of the roof fell. The material seen on the floor averaged approx. 12"-24", and amounted to an est. 100 Tns. No damage or impairment caused.

On Monday, March 19,2007-Leadman called the surface office about 8am to inform me of a roof fall at the intersection of NE 10 @ U. This area had previously been closed off to access on 2-6-07. This area was not an active mining area. The roof fall was approximately 30' wide and 40' long forming a bowl in the roof roughly 20 feet high. MSHA was immediately called.

Employee slipped and fell on his right hip and jarred his back while trying to retrieve broken drill steel from the primary hopper.

Roof fall approx. 1100" down slope from portal causing damage to steel roof supports. Roof was supported by steel sets 24' wide consisting of 12 X 65 horizontal beams and 2 ea 10 X 54 vertical legs, w/4" x 6" wood lagged and cribbed w/ 6" hardwood.

While scaling intersection of NW10-NWD, roof fell on boom causing machine to tip forward damaging right front axle housing, machine was working under bolted back, no damage to operator's cab or operator.(roof fall on scaler boom. per 220062580017)

Roof fall in N36 falling on berm and sliding out into NW-8 roadway partialy blocking access. Area was inspected and cleaned up of loose material. Area adjacent to this had been glued, bolted and strapped.

Roof fall in intersection of V6 & N34 - Sta. 98. Roof fall in N34 extended into and across V6 causing steel roof supports to collapse blocking access to North side of the mine.

Roof fall in N22- at CV2andCV3 conveyor transfer point. Damage to #2 Conv. and support stucture.

Roof fall in abandoned area of north side of the mine. Area had been previously been barricaded off due to bad roof conditions.Fall was at intersecton of 3A & S16 . Secondary escapeway was inspected and found secure .

Roof fall in A3 at station 78 .Fall area has been blocked off at both sides and alternate route has been established

Roof fall at Sta. 49 damaging Ci conveyor and the steel arch covering the conveyor.

Employee was welding overhead and a hot spawl entered his left ear.

While scaling loose rock with the scaler, rock came down on the transfer of #2 & 3 belts. The area was approx. 8 ft. thick x 25 ft. in diameter.

EE was driving anfo truck with door open. When he went around a turn the door closed fracturing his left hand.

Employee jumped approx. 4 ft. from man backet to ground - felt pain in left knee.

EE WAS INSTALLING RODS IN STEEL. A ROCK FELL OFFRIB HITTING HIS RIGHT SHOULDER AND LEFT ARM CAUSING SEPARATION OF SHOULDER.

EE WAS INSTALLING RODS IN STEEL. ROCK FELL FROM RIB HITTING HIS RIGHT ELBOW CAUSING BRUISES AND LACERATION.

EE WAS CHECKING FLUID LEVELS ON LOADER TRANSMISSION. COVER LID CLOSED ON FINGER.

MAN WAS RAISING LID ON ST8 LOADER TO CHECK FLUID LEVEL & DIDN'T GET LID LAYED BACK. AS HE REACH DOWN TO CHECK FLUID SAID HE MUST HAVE HIT LID WITH HAND. HE WAS PULLING HAND OUT WHEN LID FELL. STRUCK INDEX FINGER. HAD TO HAVE STITCHES.
