The miner climbed the ladder on the side of the aggregate feeder to dislodge some stuck material. The material dislodged causing the bin to shift. Which caused the miner to fall off of the ladder, injuring EE's left shoulder and left index finger. EE received stiches to the finger.

The miner was changing a bearing on the head pulley of a conveyor. When EE loosened the pillow block bearing the shaft popped up striking EE in the palm of EE's left hand causing a laceration which required stitches.
