THE EMPLOYEE WAS CLEANING AROUND A TAIL PULLEY IN A RESTRICTED AREA. THE GUARD FOR THE TAIL PULLEY HAD BEEN REMOVED FOR REPAIR. THE EMPLOYEE LOST HIS BALANCE AND FELL BACK THROWING OUT HIS RIG HT ARM WHICH GOT CAUGHT BY THE TAIL PULLEY. HE KICKED OUT WITH HIS LEFT FOOT WHICH WAS CAUGHT BY ANOTHER CONVEYOR.

EMPLOYEE WAS ASSISTING THE CRUSHER FOREMAN REMOVE THE TOP OF THE CYRO CRUSHER WHEN HIS RIGHT HAND WAS PINCHED BETWEEN THE SIDE OF THE CRUSHER AND THE ADJUSTING CABLE CAUSING TEMPORARY NERVE DA MAGE. THIS INJURY WAS THE RESULT OF POOR COMMUNICATION BETWEEN THE FOREMAN, THE INJURED AND THE LOADER OPERATOR, WHO WAS PULLING ON THE CABLE WITH THE LOADER.
