EE HAD A RAKE MADE OF 1/2 INCH PIPE WITH A PIECE OF ANGLE IRON WELDED ON IT SLIDE DOWN A THE SLIDE OF THE CRUSHING PLANT AND HIT HIM ON THE HARDHAT. THE PIPE WAS STANDING UPRIGHT AND ONE OF TH E OTHER EE ACCIDENTLY KNOCKED IT OVER ON THE INJURED EE. EE STATED THAT THE PIPE DID HIT HIM ON THE HARDHAT AND AT THE TIME OF THE INCIDENT HE WAS NOT INJURED JUST REPORTING IT.

EMPLOYEE HAD BACKED OFF A NUT ON APIECE OF THE CRUSHING PLANT.BEHIND THE NUT WAS A SHIM OF 1 1/4" PIPE AND BEHIND THAT A BROKEN SPRING.SOMEHOW THE SHIM HAD CAUGHT UP ON THE THREADS OF THE ROD. WHEN IT LET GO IT HIT THE EMPLOYEE IN THE FACE(1 1/4" PIPE),CAUSING A SMALL CUT IN THE UPPER LIP AND ONE BROKEN TOOTH AND A DISLOCATED TOOTH.
