EMPLOYEE WAS A ROOF BOLTER OPERATOR HE WAS BENDING A ROOF BOLT TO PUT INTO THE TOP WHILE BENDING THE BOLT HE STRAINED HIS LOWER BACK

EMPLOYEE WAS OPERATING A BATTERY POWERED SCOOTER. HE RAN INTO A SKID THAT WAS LOADED WITH HALF-HEADERS. THIS CAUSED THE STEERING WHEEL TO WHIRL AROUND AND CATCH HIS THUMB.
