Investigation is still going on at this time. Per preliminary report: Descending the coal haul road, victim began to lose brakes. He attempted to straddle an axle berm on his left. The truck ran up onto the berm for approximately 100' before it turned over. Victim was found partly out of the passenger side door beneath the right exaust stack. Unable to remove him until EMS arrived.

EE was walking from one crosscut outby the face in the #2 entry toward the face of the #2 entry when he was struck by the #1 shuttle car.

Was loading rock out of #7 face. The shuttle car was making the turn out of the entry. EE was caught between the shuttle car and the rib.

Victim died as a result of an explosion, cause unkown at this time. Received notification on 4/5/2010 that our employee was missing. Received notification late 4/6/2010 that employee was deceased.
