EMPL WAS SETTING UP FUEL OIL BARREL ROLLED BARREL OUT OF BACK HOE BUCKET ONTO GROUND TURNED IT UPRIGHT BY PICKING UP ONE END & BACK WENT OUT OF PLACE.
REPORTED TO US THAT HE WAS NOT HURT & WOULD NOT GO TO DR & BECAUSE OF THIS I DID NOT REPORT TO MSHA AT THAT TIME. ON 3/7/91 WORKERS COMP FORM FILLED OUT BY EMPL & SENT FROM DRS OFFICE. TREATME NT & CHARGES GO BACK TO 8/28/90. WAS OFF WORK FOR 6 MONTHS & DIAGNOSIS WAS HEADACHE.
EE WAS CLEANING CATS WITH CAT SPADE AND HURT LOWER BACK.
EMPLOYEE WAS TIGHTENING THE BOLTS ON THE HYDRAULIC TANK OF THE CATERPILLAR D9L. AS HE WAS PULLING ON THE WRENCH IT SLIPPED CAUSING HIM TO LOSE HIS BALANCE AND HE FELL 8 FT. TO THE GROUND.
EMPLOYEE WAS REPAIRING INTER LOCK CHAIN. IT SLIPPED AND FELL ON RIGHT HAND CAUSING CONTUSIONS AND LACERATIONS TO FINGERS.
EMPL WAS UP ON ROOF OF DRAG CHECKING FRICTION WHEN HE SLIPPED & FELL THROUGH FRAME THAT GOES ACROSS ROOF OF DRAG. WHEN HE FELL HE HIT A BOLT STICKING OUT OF FRAME.
