While adding a steel the steel fell out of the top of the loader. The operator thought they were able to get the steel back into the loader properly. The operator then threaded onto the steel to make the next connection. The steel came free from the rotary head & dropped approx. 2ft onto the drill table & then slid off the table & dropped 3-4ft striking the operator's ankle/foot.
