Employee was operating a Euclid Haul Truck transporting material from the pit to the crusher when towards the end of his shift hit a pot hole and felt a pop in his lower back. He was feeling a sharp pain run down his back side and down his legs.

The power went off in the tunnel feeder. He went down to investigate and all power was off including the lights. I started to head back up to the control room and stepped in the sump pump hole and fell down. The cover was removed to clean the hole out but never replaced.

While working on G cone crusher conveyor a wire screen guard which was wedged between conveyor and spilled material became loose as material was removed and fell approx. 2 feet and hit his left knee leaving a small puncture and bruise.
