Employee was operating a Euclid Haul Truck transporting material from the pit to the crusher when towards the end of his shift hit a pot hole and felt a pop in his lower back. He was feeling a sharp pain run down his back side and down his legs.

The power went off in the tunnel feeder. He went down to investigate and all power was off including the lights. I started to head back up to the control room and stepped in the sump pump hole and fell down. The cover was removed to clean the hole out but never replaced.

FAILURE TO LOWER DUMP BED ON 773 HAUL TRUCK, RESULTING IN HITTING ALMASCHOOL BRIDGE. INJURY CAUSED BY SAFETY GLASSES CUTTING HIS NOSE DUE TO CONTACT WITH THE STEERING WHEEL.

OPERATOR HAD DRY MATERIAL IN THE SKID STEER BUCKET, WHEN OPERATOR DUMPED THE BUCKET WIND WAS BLOWING TOWARD HIM AND DUST GOT INTO HIS LEFT EYE. MEDICAL CLINIC PUT NO RESTRICTIONS. CLINIC CLEAN ED EYE AND USED ANTIBIOTICS.

EE DROVE TRUCK INTO A OVERPASS WITH BED FULLY RAISED AT APPROX 20-30 MPH. HE BROKE WINDSHIELD, SHIFTER COLUMN AND STEERING WHEEL WHILE BOUNCING AROUND IN CAB. HE RECEIVED 6-8 INCH LACERATION T O HIS HEAD, AND POSSIBLE BUMPS AND BRUISES.

LABORER AT WASH PLANT WAS CLEANING UP AROUND THE 1" TRANSFER BELT WHEN HE BRUSHED UP AGAINST ELECTRICAL LINE HANGING FROM THE HEAD PULLEY OF THE1" TRANSFER BELT TO THE TAIL PULLEY OF THE 01 ST ACKER. THE INSULATION ON THE 220 VOLT LINE WAS DENERGIZED CAUSING A SHOCK TO EMPLOYEE AS HE MADE CONTACT WITH THE LINE.
WHILE CUTTING A V BELT, LEFT MIDDLE FINGER WAS TRAPPED BETWEEN THE COUNTERWEIGHT AND SHAKER SCREEN. LOOSENED V BELT AND THE COUNTERWEIGHT SHIFTED, CUTTING FINGER.
