While unfastening a bolt in order to remove a damaged steel fabricated guard, a sudden unexpected movement of the guard striking the miner's left hand middle finger causing a fracture and laceration of the distal and middle phalanx.

Employee stepped down from drill cab, released grip on hand hold as left foot touched ground, left ankle rolled over on a stone causing him to lose balance and fall landing on left shoulder. Left ankle swelling caused difficult walking. Left shoulder popped as it hit ground, causing it to be very sore and unable to lift arm without pain.

RIGHT HAND FINGERS #3, 4 & 5 WERE LACERATED, 19 TOTAL STITCHES, AND FINGER #4 WAS FRACTURED CAUSED BY PINCHING IN BETWEEN TWO 250 LB FEEDER PLATES. WHILE PRYING INTO POSITION, A PLATE MOVED IN UNEXPECTED DIRECTION SUDDENLY. EE HAD JUST WARNED CO-WORKER TO KEEP HIS HADN OUT OF PINCH POINT.

SHORTLY AFTER START OF AFTERNOON PRODUCTION OPERATION A LARGE OBLONG ROCK WAS STUCK IN PRIMARY JAW CRUSHER HOPPER NORMAL PROCEDURES WERE FOLLOWED BY CRUSHER OPERATOR WHO USED ELECTRIC HOIST & A CHAIN WRAPPED AROUND ROCK TO PULL THE 4ROCK UPONTO THE BUILDING FLOOR IN ORDER TO REPOSITION THE ROCK & REINTRODUCT IT INTO THE HOPPER HAVING PULLED THE ROCK ONTO EDGE OF BUILDING FLOOR VICT
