EMPLOYEE WAS MAKING A DELIVERY TO THE B.S. #4 MINE, BACKED TRUCK UP TO LOADING DOCK, GOT IN THE BED OF THE TRUCK TO REMOVE THE REAR TAILGATE. THEN PLACED THE TAILGATE ON THE SIDE OF TRUCK BED RESTING AGAINST SIDE GATE ON THE TRUCK. IN THE PROCESS OFF GETTING OFF OF THE REAR OF THE TRUCK EMPLOYEE PLACED HIS RIGHT HAND ON THE TAILGATE THAT HE HAD PLACED ON THE SIDE AS HE WAS STEPPING

EMPLOYEE WAS UNLOADING THE SHOP SUPPLY TRUCK AT THE #5 MINE HE HAD SET A PIECE OF GRATING ON THE DOCK WITH THE BOOM CRANE ON THE SUPPLY TRUCK ONE END OF THE GRATING WAS RESTING ON A PULLEY O N THE GROUND HE THEN STARTED TO PUSH THE GRATING OFF OF THE PULLEY AND IT SLIPPED OFF AND THE GRATING SWUNG AROUND AND HIT ON THE TOP OF THE RT FOOT RESULT OF INJURY WAS A FRACTURED 2ND METAT

EMPLOYEE WAS WORKING IN THE MACHINIST AREA OF THE SHOP, WAS IN PROCESS OF LOOSENING A NUT WITH A CRESCENT WRENCH, THE WRENCH SLIPPED OFF OF THE NUTCATCHING THE EMPLOYEES FINGER BETWEEN THE WRE NCH AND A PIECE OF ANGLE. RESULT OF THE INJURY WAS A FRACTURE TO THE TIP OF HIS LITTLE FINGER ON THE LEFT HAND.
