ON THE 2NORTH SECTION IN THE #6 ENTRY AT #14 BLOCK THE EE STEPPED ON A PIECE OF ROCK OR COAL THAT WAS HIDDEN BY MUD CAUSING HIM TO TWIST HIS LEFT ANKLE RESULTING IN A SRAIN. HE ALSO HAD SOME P AIN UP HIS LEFT LEG.
ON THE 2-NORTH SECTION, #4 ENTRY THE EE WAS ROOF BOLTING ON THE RT SIDE OF A TWINHEAD BOLTER. THERE WAS A SLIP RUNNING THRU THE TOP AND AS HE WAS POSITIONING THE DRILL STEEL TO START DRILLING THE HOLE A PIECE OF ROOF ROCK FELL OUT OF THE TOP STRIKING HIM ON THE TOP OF THE RT HAND THUMB, CATCHING THE THUMB BETWEEN THE ROCK AND THE BOLTER DRILL HEAD.
A ROOF FALL WAS FOUND IN THE #5 ENTRY OF THE MAIN WEST ANEX OF THE MINE AT SPAD #804, IN AN INTERSECTION. THE FALL WAS 16'WIDE, 30'LONG, & 5'HIGH. THE AREA WAS BOLTED WITH 5' COMBINATION BOLTS . NO INJURIES OCCURED.
A ROOF FALL WAS FOUND IN THE 2 RETURN ENTRY AT SPAD 782 OF THE MAIN WEST AREA OF THE MINE. IN THIS AREA 1,2, AND 3 ENTRIES ARE RETURNS THE FALL WAS 4 TO 5' HIGH, 16'WIDE AND 22' TO 25'LONG. TH ERE WAS A SLIP RUNNING THRU THIS AREA. THE AREA WAS BOLTED WITH 5' CONBINATION BOLTS. NO INJURIES OCCURED.
A ROOF FALL WAS FOUND IN THE #1 ENTRY (RETURN) OF THE MAIN WEST SECTION AT SPAD #755. THE FALL WAS 5 1/2' HIGH, 17' WIDE AND 18' LONG. THE AREA HAD BEEN BOLTED WITH 5' COMBINATION BOLTS. NO IN JURIES OCCURRED.
THE EE WAS RIDING IN AN OPEN MANTRIP AND THE TROLLEY POLE CAME OFF THE TROLLEY WIRE AND CAUGHT A TROLLEY WIRE SUPPORT ROPE CAUSING THE TROLLEY POLE TO PULL OUT OF ITS BASE. THE TROLLEY POLE HA RP THEN STRUCK THE EE ON THE RIGHT HAND NEAR THEBASE OF THE LITTLE FINGER PINCHING THIS ARE OF THE HAND BETWEEN THE METAL WINDOWFRAME ON THE JEEP AND THE HARP.
A ROOF FALL OCCURRED ON THE MAIN WEST SUPPLY TRACK AT #8 BLOCK. THE FALL WAS 5' HIGH, 18' WIDE, AND 40' LONG. THE AREA WAS BOLTED WITH 5' COMBINATION BOLTS. NO INJURIES OCCURRED.
ON THE 1 NORTH SECTION IN 3 ENTRY AT 9 BLOCK A ROOF FALL OCCURRED THE FALL WAS 5' HIGH, 20' LONG AND 15' WIDE. THE AREA HAD BEEN BOLTED WITH 5' CONBINATION BOLTS. NO INJURES OCCURRED.
ON THE MAIN LIKE HAULAGE BETWEEN 60&00 AND 65&00. THE EE WAS OPERATING A TRUCK CHEMOS. WHEN UNCOUPLING A LOADED CAR FROM THE TRACK CLEANER. THE EE PICKED UP ON THE UNCOUPLING HANDLE HE FELT A PULLING SENSATION IN THE BICEP PAIN OF THE LEFT ARM.
ON THE 1 NORTH SECTION THE EE WAS HELPING MOVE A ROOF BOLT ON FROM 2 ENTRY TO 3 ENTRY. WHILE HANDLING THE POWER CABLE, HE STEPPED IN A HOLE IN THE MINE FLOOR CAUSING PAIN IN THE LEFT KNEE WHIC H WAS DIAGNOSISED AS A SPRAIN.
TWO EE'S WERE MAKING A BOLT SPLICE AS THE INJURED EE WAS MOVING THE TEMPLATE, THE OTHER EE CONT TO SWING THE AXE AND STRUCK INJ EE'S INDEX FINGER ON HIS RIGHT HAND NEAR THE TIP OF THE FINGER CAUSING A LACERATION AND A NON-TREATABLE FRACTURE TO THE TIP OF THE FINGER.
EE GLOVE BECAME CAUGHT ON A FRAYED WIRE THAT WAS BEING ROLLED ONTO A REEL. THIS CAUSED HIS HAND TO BE PULLED INTO THE REEL, PINCHING HIS THUMB BETWEEN THE WIRE.
A ROOF AL OCCURED ON THE MAIN WEST SECTION IN THE #4 ENTRY BELT LINE AT #21 BLOCK. THE FALL WAS 5 FEET HIGH, 35 FEET WIDE AND 30 FEET LONG. THE AREA HAD BEEN BOLTED WITH 5 FOOT COMBINATION BOL TS. NO INJUIRES OCCURED.
AT THE MAIN WEST SECTION IN THE INTERSECTION OF #5 ENTRY,OUTBY THE POWER CENTER THE EMPLOYEE WAS HELPING CARRY A 4"X30' ALUMINUM WATER LINE WHEN HE STEPPED IN THE MUD,SLIPPED AND FELL STRIKING HIS LEFT KNEE ON THE MINE FLOOR CAUSING PAIN IN THE LEFT KNEE AND UPPER BACK.
INJURED WAS WORKING TOGETHER WITH ANOTHER FOREST CONSTRUCTION EMPLOYEE TO REMOVE CROSSARM FROM UTILITY POLE WHEN ACCIDENT OCCURED. INJURED WAS STANDING ON GROUND WHEN CROSSARM FELL FROM BUCKET OF AERIAL LIFT TRUCK,STRIKING HIS L.WRIST.
ON THE MAIN WEST SECTION, X-CUT #6 TO #7, EE WAS HANDLING THE MINER CABLE AS THE MINER WAS BEING BACKED UP. HE STEPPED IN THE MUD & BOTH FEET SLIPPED CAUSING A JERKING MOTION CREATING PAIN IN THE RIGHT SIDE OF THE LOWER BACK & NUMBING DOWN THE RIGHT LEG TO THE FOOT.
ON THE MAIN WEST SECTION, WAS PICKING UP HIS TOOL BAG. USING A TWISTING MOTION TO PLACE IT ON HIS RIGHT SHOULDER HE FELT PAIN IN THE RIGHT SIDE OF HIS LOWER BACK AND SOME NUMBNESS IN HIS RIGH T LEG.

FURTHER RM EDICAL DIAGNOSS INDICATED THATHE HAS A ROTOR CUFF TAR WHICH IS CAUSED BY REPETITIVE OVRHEAD ARM MOVEMENT SIMILAR TO SWIMMING, WHICH IS ONE OF HIS HOBBIES.

15 EAST SUPPLY TRACK. MAN WAS RIDING IN A MOTOR.HE LOOKED INTO THE X CUT FOR SOME LAY FLAT WATERCLINE. WHEN HE TURNED AROUND HE STRUCK HIS HEAD ON THE MINE ROOF. FRACTURED C-5 VERTEBRAE.
185 TOO MAINLINE HAULAGE. THE POLE ON HIS MOTOR CAME OFF OF THE WIRE. WENT OVER & HIT A CRIB. IT BOUNCED BACK & STRUCK HIM ON THE HEAD. WHEN HISPOLE CAME OFF HE HIT THE POLE RETRACTOR BUTTON & PUT HIS FOOT ON THE SWITCH TO DEACTIVATE THE PLLE LIMITOR. THIS CONTRIBUTED TO HIM BEING STRUCK IN HEAD. RECEIVED A CONTUSION.
14 EAST A STEEL WAS STUCK IN ROOF & HIS HELPER WAS REPOSITIONING THE BOLTER FOR STEEL CHUCK ALIGNMENT THE INJURED HAD HIS HAND ON THE STEEL HIS HELPER MOVED THE MACHINE TOO MUCH & HIS HAND WA S CAUGHT BETWEEN THE STEEL & THE STRUCTURE HE RECEIVED A BLOOD BLISER UNDER THE THUMB NAIL OF THE RT THUMB
EE THE INJURED ALONE WITH 3 OTHER EMPLOYEES WAS DRAGGING 3 FT X20FT PIECE OF METAL GRATING ACROSS 4IN I BEAMS HIS FINGER WAS CAUGHT BETWEEN THE GRATING & 4IN I BEAM & THE FORCE OF MOVING THE G RATING CAUSED AN AVULSION TO THE LEFT INDEX FINGER WHICH NEEDED 4 STITCHES TO CLOSE

16 EAST THERE WAS AN IGNTION OF METHANE AT A BOLT HOLE WHILE THE BOLTING OPERATION WAS IN PROGRESS.THE BOLT HOLE WAS APPROXIMATELY 16 FT FROM THE FACE.

16 EAST TRCK. EE WAS SITTING ON GROUND IN A SPUR ON 16 EAST SECTION SKINNING OUT A PIECE OF CABLE. WITH OPEN KNIFE IN HAND, HE REACHED BACK TO REPOSITION HIMSELF AND THE KNIFE SLIPPED AND CUT HIS RT FOREFINGER. RECEIVED A INCISION REQUIRING4 STITCHES TO CLOSE.
15 EAST LIGHT CORD CAUGHT TIMBER JACK LEVER WHILE HE WAS BLOCKING OVER TOP OF A PLANK & THE LIGHT CORD PULLED HIS HAT OFF. A BOARD FELL OFF OF THE PLANK & STURCK HIM ON THE HEAD.
EE WAS HELPING MECHANIC C WIREMEN CHANGE OXYGEN ACETYLENE. TANICS IN TRACK BUS AND FELT A SHARP PAIN IN LOWER MIDDLE PART OF BACK.
MT. MORRIS ELEVATOR DOWN FROM 8:15 AM TO 9:20 AM WITH 3 PEOPLE ON THE CAGE BECAUSE OF MALFUNCTIONING DOOR LOCK SWITCHES. MEN WERE OFF OF THE ELEVATOR AT 8:30 AM.
EMPLOYEE WAS LOWERING CLOTHES BASKET IN BATH HOUSE AT MT. MORRIS. THE "S" HOOK OR CHAIN ON BASKET WRAPPED AROUND INDEX, MIDDLE & RING FINGERS. NO DISPLACED FRACTURE TO MIDDLE FINGER.
EE CARRYING GARBAGE TO DUMESTER AND SLIPPED ON ICE. FEET WENT OUT FROM UNDER HIM. TO BREAK THE FALL, HE STUCK HIS RT HAND OUT TO CATCH HIMSELF AND SPRAINED HIS RT WRIST.

ON 1-1-95, AS HE WAS CHANGING A TILT JACK, HE AGAIN STRAINED HIS BACKAND STATED THAT HE MUST HAVE RETURNED TO WORK TOO SOON AFTER THE ORIGINAL INJURY. HE IS NOW SEEKING MEDICAL INTERVENTION.

7 NORTH SUPPLY TRACK, INJURED WAS RIDING IN A JEEP WHICH RAN INTO A BROKEN DOWN MOTOR. HE RECEIVED A WHIP LASH EFFECT TO HIS BACK MIDDLE AND LOWER, NECK AND RIGHT HIP. THE RESULTING INJURY W AS A STRAIN/SPRAIN.
INJURED WAS OPERATING A FOURWHEEL DRIVE RUBBER TIRED TRACTOR MOWING RIGHT-OF-WAY FOR POWER LINE. HE WAS PROGRESSING ALONG A GRADE OF APPR0X. TWENTY PERCENT WHEN THE TRACTOR'S RIGHT FRONT WHEEL RAN UPON AN UNSEEN STUMP CAUSING THE TRACTOR TO ROLL OVER TO DRIVER'S LEFT SIDE. IT ROLLED OVER TWICE LANDING ON ITS WHEELS ON THE FLAT BELOW.
UPON ENTERING MINE PROPERTY WHILE CROSSING RAIL ROAD TRACKS TRUCK WAS STRUCK ON RIGHT HAND SIDE WAS PUSHED DOWN THE TRACKS APPROX 100 FT AND THEN WAS PUSH OFF TRACKS AND CAME TO REST ALONG SID E RAILROAD TRACKS TRUCK FACING AWAY FROM TRACKS.

EE WAS BUILDING CRIBS & WHILE HANDLING A CRIB BLOCK WHICH WAS WET, IT SLIPPED OUT OF HIS HAND & STRUCK HIM ON THE KNEE. DIAGNOSED AS A CONTUSION TO THE RT. KNEE.
THE AIRLOCK DOORS AT THE FACE WERE BEING REMOVED AND THE HINGE PIN FOR THE DOOR WAS BEING PULLED OUT. THE COTTER PIN WAS DROPPED AND HE WAS LOOKING FOR IT. THERE WERE BLOCKS FROM AN OLD STOPPI NG STACKED ALONG THE INBY RIB WHERE THE COTTER PIN HAD FALLEN. AS HE WAS LOOKING FOR THE PIN, A CEMENT BLOCK FELL FROM THE WALL OF THE DOOR AND CAUGHT HIS FINGER BETWEEN THE FALLING BLOCK AND
ELEVATOR WAS NOT FUNCTIONING ON 8-25-94 FROM 3:30 PM TO 6:30. NO ONE WAS ON THE ELEVATOR AT THE TIME OF MALFUNCTION. THE BUTTON LIMIT SWITCH CAUSED THE PROBLEM.
ELEVATOR DOWN ON BOTTOM WITH NO ENTRAPMENT. WATE RIN JUNCTION BOX ON BOTTOM AND WIRE FEEDING BUFFER SWITHC BURNT IN TWO. BOWERS ELEVATOR.
3 EAST 1 CT INJURED WAS HELPING UNLOAD A TOOL BOX FROM A JEEP WHEN HIS FOOT WAS CAUGHT BETWEEN THE TURN RAIL AND STRAIGHT RAIL OF A TRACK SWITHC CAUSING HIM TO FALL AND FRACTURE HIS RIGHT FOOT . THE FRACTURE WAS A HAIRLINE FRACTURE TO A BONE IN THE ARCH OF THEFOOT.
THE BOWER ELEVATOR WAS IN OPERABLE FROM 12:20 AM UNTIL 2:05 AM MICCRO SWITCH NOT MAKE UP.
15E SUPPLY HELPIN GCARRY 4" WATERLINE AND FELT PAIN IN LT SIDE. PULLED MUSCLE IN LT ABDOMEN.
AN UNPLANNED ROOF FALL OCCURRED ABOVE THE ANCHORAGE ZONE IN 14 EAST RETURN ENTRY AT #115 BLOCK FALL IS APPROX. 16' X 16' X 12' HIGH. NO PEOPLE OR EQUIPMENT WERE INVOLVED.
13 EAST LONGWALL RECOVERY A SHIELD SLID OFF CRIB BLOCK ON DUCK BILL OF SCOOP CATCHING HIS LEGS BETWEEN ROOF BEAM AND COAL SPILLAGE. RECEIVED A FRACTURE TO BOTH LOWER LEGS.
ELEVATOR WAS INOPERABLE FROM 1515 TO 1645 ON 7-19-94. THE BOTTOM DOOR SWITCH CAUSED THE PROBLEM. NO ONE ON CAGE AT THE TIME OF MALFUNCTION.
