REPORTED BUT SAID HE WAS FINE 12-26-01, WANTED TO GO TO DR TO HAVE BACK LOOKED AT. ON THURS 11-8EMPLOYEE WAS CLIMBING DOWN LADDER FROM CRUSHER, HE TURNED AND JUMPED DOWN LAST 2 RUNGS TWISTING HIS BACK.

EMPLOYEE WAS TRYING TO TAKE FILLER CAP OFF OF THE 450 EXCAVATOR, STRUGGLING BECAUSE IT WAS TIGHT. WHEN IT FINALLY LOOSENED HE WAILED HIS LEFT ARM (ELBOW AREA) AGAINST THE RAIL ON THE TOP OF THE MACHINE.
