Drilling holes in ceiling by hand causing injury to right arm and shoulder.
Employee was moving a 55 gal drum of oil in the shop. Employee slipped on a 5-gal bucket lid. Employee fell backward onto floor. Drum fell beside his leg. Causing bruising on leg and back strain.
After climbing off of the front end loader to move some vehicles the employee slipped on ice and fell to the ground.

Truck #2 while backing up to the dump area, the trucks braking systems failed and the truck began rolling foward. The truck traveled down the road approx. 50 yard and the driver jumped out, the truck cont. down the road about 40 yard hit the berm and then turned onto its right side.
