EMPLOYEE WAS SHOVELING SPOIL FROM AROUND THE AUGER.ANOTHER EMPLOYEE WAS OPERATING THE AUGER AND HE WENT TO PULL THE AUGER OUT OF THE HOLE,THE WHIPLINE CABLE BROKE ABOVE THE HEADACHE BALL AND F ELL KNOCKING OFF EMPLOYEE'S HARDHAT STRIKING HIM IN THE HEAD.

Miner was using a heated water pressure washer to clean a transmission and while relocating the hose, EE shot hot water down into EE's boot. EE suffered a second degree burn to the ankle area and the skin peeled off.

A miner who was involved in metal plate cutting activities went to readjust the PPE welding sleeves on right arm, and accidentally contacted the arm with the cutting torch, resulting in a burn.

Employee was installing V-belts on 4 1/4 crusher.  The employee was using a pipe wrench to turn the shive to get the v-belts to fall into place in the groves and as the pressure released from the pipe wrench, the chive rolled quickly pinching left index finger between the wrench and the chive guard.

Employee states that he carried 5 pails (5 gallon) of oil up a flight of stairs to perform a PM on a haul truck and noticed a stinging sensation in his lower back. His physician has ordered physical therapy for his lower back and taken him out of work for 4-6 weeks. He has just put us on notice of the injury. Employee did not report at the time of injury.

EE had right thumb caught between steel beams while replacing center section of floor following crusher repairs. As it was lowered into place a weld broke and the front section, consisting of a 12" x 112" C chanel and ass. handrail, swung down and crushed EE's right thumb against the structure. The piece then fell and hit the hand rails.

EE was replacing a bucket tooth and shank that had broken off the dragline bucket. He lifted the approx 120lb assembley several times and reported back and groin pain the next day. He was diagnosed with a hernia which would require surgery to repair. He worked until 6/26/07 and that is when he had surgery.. First day lost is 6/26/07. Estimated lost time 4-6 weeks.

EE was operating loader cleaning in the plant when he hit an underground drain pipe. Loader stopped abruptly and he was thrown forward causing his left thumb to bend backwards towards the wrist. He felt some pain in it which continued to get worse until the 23rd when he went to the Dr. X-ray revealed a fracture of a small bone in the left wrist. Medical only. No lost time.

EE WAS CUT ON FACE BY FLYING METAL CHIP. HE WAS REPLACING TOOTH WHEN HE HIT THE TOOTH WITH A 20#HAMMER TO TIGHTEN IT SO HE COULD GET THE LOCKING WEDGE IN. A PIECE OF METAL CAME OFF AND STRUCK THE RIGHT SIDE OF HIS FACE ON THE CHEEK BONE. A SLIVER 1 1/4" LONG EMBEDDED IN HIS CHEEK. HE WAS INSPECTING THE WOUND WHEN HE NOTICED THE METAL WAS STILL THERE. E.R. REMOVED & GAVE 4 SUTURES.

EE WAS CHECKING END CAP BOLTS ON PRIMARY FEEDER SHAFT WHEN HE SLIPPED AND STARTED TO FALL, GRASPING DRIVE BELT WITH BOTH HANDS, RESULTING IN THE WEIGHT OF HIS BODY CAUSING THE BELTS TO ROTATE ON SHEAVE, WHICH FED BOTH HANDS AROUND THE RADIUS OF THE SHEAVE, TRAPPING HANDS BETWEEN THE SHEAVE AND BELTS. RESULTED IN TWO FRACTURED FINGERTIPS AND LACERATIONS TO BOTH HANDS.
