CLARKS QUARRY: 1 recorded fatality Metal/Non-Metal

A miner who was involved in metal plate cutting activities went to readjust the PPE welding sleeves on right arm, and accidentally contacted the arm with the cutting torch, resulting in a burn.

A miner was clearing limbs out of a tree, when a limb bounced off the ground toward the miner, striking them in the face and resulting in stitches.

A miner received treatment for rust ring in the right eye, and was issued a prescription.

Employee was removing an engine side panel off a D7E and felt something pull in back. Diagnosed with a pinched nerve.

Employee was installing V-belts on 4 1/4 crusher.  The employee was using a pipe wrench to turn the shive to get the v-belts to fall into place in the groves and as the pressure released from the pipe wrench, the chive rolled quickly pinching left index finger between the wrench and the chive guard.

Employee states that he carried 5 pails (5 gallon) of oil up a flight of stairs to perform a PM on a haul truck and noticed a stinging sensation in his lower back. His physician has ordered physical therapy for his lower back and taken him out of work for 4-6 weeks. He has just put us on notice of the injury. Employee did not report at the time of injury.

Laceration to lip from metal chip generated by hammering a dragline bucket tooth onto the bucket lip with a 10 lb. hammer. Cut required 8 stitches to close.

EE had right thumb caught between steel beams while replacing center section of floor following crusher repairs. As it was lowered into place a weld broke and the front section, consisting of a 12" x 112" C chanel and ass. handrail, swung down and crushed EE's right thumb against the structure. The piece then fell and hit the hand rails.

Back started hurting while loading 1st of 2 shots but continued loading & carrying 50# bags and 4x20 sticks of blastex.

EE was replacing a bucket tooth and shank that had broken off the dragline bucket. He lifted the approx 120lb assembley several times and reported back and groin pain the next day. He was diagnosed with a hernia which would require surgery to repair. He worked until 6/26/07 and that is when he had surgery.. First day lost is 6/26/07. Estimated lost time 4-6 weeks.

EE was operating loader cleaning in the plant when he hit an underground drain pipe. Loader stopped abruptly and he was thrown forward causing his left thumb to bend backwards towards the wrist. He felt some pain in it which continued to get worse until the 23rd when he went to the Dr. X-ray revealed a fracture of a small bone in the left wrist. Medical only. No lost time.

EE SUFFERED LACERATIONS AND ABRASIONS WHEN ROCKBREAKER WAS STRUCK BY BIG ROCKS FALLING FROM THE HIGHWALL. EE WAS OPERATING A TYLEDYNE HAMMER MOUNTED ON 988B LOADER APPROX. 35' FROM A 35' HIGHW ALL, WHEN A SECTION OF WALL FELL AND STRUCK THE LOADER.

EE WAS CUT ON FACE BY FLYING METAL CHIP. HE WAS REPLACING TOOTH WHEN HE HIT THE TOOTH WITH A 20#HAMMER TO TIGHTEN IT SO HE COULD GET THE LOCKING WEDGE IN. A PIECE OF METAL CAME OFF AND STRUCK THE RIGHT SIDE OF HIS FACE ON THE CHEEK BONE. A SLIVER 1 1/4" LONG EMBEDDED IN HIS CHEEK. HE WAS INSPECTING THE WOUND WHEN HE NOTICED THE METAL WAS STILL THERE. E.R. REMOVED & GAVE 4 SUTURES.

EE SUSTAINED A HERNIA WHILE PULLING A WRENCH TO TIGHTEN BOLTS ONTHE WASH SCREEN. HE WAS WORKING ON THE NIGHT SHIFT AT THE WASH SCREEN WHILE PULLING ON THE WRENCH HE FELT A PAIN IN HIS ABDOMINA L REGION. HE REPORTED THE INJURY TO OTHER EE, LEADMAN DID NOT NOTIFY THE FORMAN, ANOTHER EE UNTIL 2-12-02. EE REPORTED TO WORK ONT HAT DATE WITH A NOTE FROM HIS OWNER PUTTING HIM ON LIGHT DUT

OTHER EE, ANOTHER EE, AND EE WERE INT HE PROCESS OF TRYING TO KNOCK A WEDGE OUT OF A DRUM. OTHER WAS HOLDING A BAR ON THE WEDGE AND EE WAS USING A SLEDGE HAMMER TO HIT THE BAR. ON ABOUT THE TH IRD HIT EE GRAZED THE BAR CAUSING THE HAMMER TO SWING TOWARD OTHER. EE TRIED TO HOLD BACK THE SWING SO THAT HE WOULD NOT HIT OTHER. THIS CAUSE EE TO TWIST HIS BACK, RESULTING IN THE BACK INJU

EE WAS STANDING ON DIVIDER DOOR WELDING TUB WHEN FOOT SLIPPED RESULTING IN FALL. THROUGH DISCHARGE CHUTE INTO CRUSHER FEED BOX RESULTING IN DISLOCATED SHOULDER, RIGHT SIDE AND CHIPPED THE BONE ON THE BALL OF THE HAMEROUS.

EE WAS USING THE HANDLE END OF A 4 LB HAMMER TO PUSH PANEL ONTO POSITION WHEN HANDLE MASHED BETWEEN HANDLE AND SCREEN WIRE. INJURY CONSISTED OF FRACTURE TO FIRST DIGIT OF LITTLE FINGER, RIGHT HAND, ALSO REQUIRING STITCHES.

EE WAS EXITING THE MACHINE FROM UNDERNEATH, HIT HIS HEAD ON STEPS KNOCKING HIS HARD HAT OFF AND LACERATING HEAD. INJURY REQUIRED SUTURING AT THE LOCAL HOSPITAL.

EE WAS PULLING A WATER HOSE AROUND TO WASH HIS 773 TRUCK, AND HURT HIS BACK.

LACERATION OF RIGHT HAND, MIDDLE FINGER Y

EE WAS CHECKING END CAP BOLTS ON PRIMARY FEEDER SHAFT WHEN HE SLIPPED AND STARTED TO FALL, GRASPING DRIVE BELT WITH BOTH HANDS, RESULTING IN THE WEIGHT OF HIS BODY CAUSING THE BELTS TO ROTATE ON SHEAVE, WHICH FED BOTH HANDS AROUND THE RADIUS OF THE SHEAVE, TRAPPING HANDS BETWEEN THE SHEAVE AND BELTS. RESULTED IN TWO FRACTURED FINGERTIPS AND LACERATIONS TO BOTH HANDS.

EMPLOYEE GOT OUT OF BULK TRUCK, TOOK SEVERAL STEPS AND STEPPED ON A ROCK. AS HE STEPPED ON THE ROCK HE TWISTED HIS ANKLE. WE WERE NOT NOTIFIED UNTIL 11-18-96.

EMP WAS USING END WRENCH ADJUSTING ALIGNMENT OF CONVEYOR BELT WHEN WRENCH SLIPPED AND PINCHED MIDDLE FINGER OF LEFT HAND BETWEEN WRENCH AND CONVEYOR FRAME RESULTING IN LOSS OF FINGERNAIL. EMP WAS NOT WEARING GLOVES AT TIME OF ACCIDENT.

USING RAZOR KNIFE CUTTING SECTION OF OLD CONVEYOR BELT WITHE FRAMING SWUARE AS STRAIGHT EDGE. EMP WAS CUTTING AWAY FROM HIMSELF. KNIFE POINT CAME OUT OF CUT IN BELT AND RODE UP ON EDGE SQUARE ACROSS LEFT INDEX FINGER FROM SECOND KNUCKLE TO FINGER TIP, RESULTING IN 11 STITCHES.

TRYING TO PUT HOOK ROLLER FRAME IN PLACE USINGT 2 CHAIN HOIST. FRAME HUNG UP ON TURN TABLE AS LET L SIDE DOWN. SLACK CAME OUT OT CHAIN AND FERKED HANDLE UP IN FACE BREAKING GLASSES

EMPLOYEE WAS HELPING OTHER EMPLOYEE CHANGE A BROKEN HYDRAULIC HOSE, HE ALLEGED WHEN HE WAS BREAKING A BOLT LOOSE, HIS KNUCKLE ON HIS THUMB, (RIGHT HAND) STRUCK BODY BO THE VEHICLE, SCRAPPING H IS KNUCKLE.
