EMPLOYEE WAS IN THE PROCESS OF CHANGING TIRES ON RIMS. AS EMPLOYEE GRABBED A TIRE AND RIM, THE TIRE AND RIM SLIPPED AND PINCHED THE EMPLOYEE'S RIGHT INDEX FINGER BETWEEN THE TIRE AND RIM.
EMPLOYEE WAS PROCEEDING TO GREASE THE TOP PORTION ON THE RADIAL STACKER. EMPLOYEE LAID DOWN ON THE BELT IN ORDER TO RIDE UP TO THE TOP. WHEN THE BELT STARTED, THE EMPLOYEE REACTED BY SITTING. UP. EMPLOYEE WAS STRUCK ON THE BACK AND NECK BYA CROSS BAR GOING OVER THE BELT.
EMPLOYEE KNOCKED A PIECE OF ANGLE IRON LOOSE WITH HIS FOOT. AS HE WAS DOING THIS A PIECE OF #9 WIRE WHICH AS LAYING NEXT TO THE PIECE OF ANGLE IRON PUNCTURED THE EMPLOYEE'S LEFT LEG CALF. EMPL OYEE RECEIVED STITCHES AT THE HOSPITAL.
