EE WAS LOOSENING BRAKES ON PUP TRAILER. THE WRENCH SLIPPED AND HE BUMPED HIS HEAD ON THE TRAILER FRAME. SINCE THIS INCIDENT. HE HAS EXPERIENCED HEADACHE. HE IS SEEING A DR FOR THIS FOR THE FIR ST TIME TODAY.
EMPLOYEE WAS MOVING BARGE ANCHORS. WHEN HE RELEASED THE WHINCH BRAKE, HE WAS HIT BY THE WINCH CRANK. HE HAD NUMEROUS ABRASIONS AND BRUISES TO BOTH ARMS. HIS LF. WRIST/ARM WAS BROKEN IN 2 PLACE S.

EMPLOYEE WAS IN THE PROCESS OF CHANGING TIRES ON RIMS. AS EMPLOYEE GRABBED A TIRE AND RIM, THE TIRE AND RIM SLIPPED AND PINCHED THE EMPLOYEE'S RIGHT INDEX FINGER BETWEEN THE TIRE AND RIM.
EMPLOYEE WAS PROCEEDING TO GREASE THE TOP PORTION ON THE RADIAL STACKER. EMPLOYEE LAID DOWN ON THE BELT IN ORDER TO RIDE UP TO THE TOP. WHEN THE BELT STARTED, THE EMPLOYEE REACTED BY SITTING. UP. EMPLOYEE WAS STRUCK ON THE BACK AND NECK BYA CROSS BAR GOING OVER THE BELT.
