Employee was exiting Skid Steer and hit EE's head on plant staircase. Closed head injury. Contusion and laceration.

Liberty Materials, Inc. · Caught in, under or between a moving and a stationary object

The silo bucket elevator broke down with loaded buckets. The maintenance crew began working on the elevator and when the backstop and tensioner were removed the buckets rolled slightly and a come-a-long smashed the injured employees hand against the structure. The employee received a deep cut to the left index finger and knuckle.

Four employees were carrying a steel pipe. Two of them dropped one end of the pipe and the pipe landed on one of their feet. The impact broke the miner's right pinky toe. The miner was wearing steel toe boots.
