An unplanned roof fall occurred on the 002-0 section, two breaks inby spad # 73724 starting in the #6 face into the #6 to #7 cross-cut. Dimension on the fall was 40' in length, 5' to 7' in width and 8' in height.

An unseen roof condition above test hole height 150 feet inby spad number 73124 in entry number 5. The fall was 22 feet in length 19 feet wide and 8 feet high.

EE was cutting trees for powerline. He had been cutting trees all day. He cut one small tree not foreseeing pressure from another tree pushing on the tree he was cutting. The saw kicked back chain brake was working correctly but apparently the angle he was cutting at prevented his arm from hitting the brake.
