A mechanic was building a tail pulley guard and needed to trim some belting to fit properly. EE leaned against the side of the conveyor and started cutting excess material with EE's utility knife. EE was cutting away from self. EE's hand slipped causing the knife to come into contact with right index finger. This was just brought to my attention that this was MSHA, not OSHA.

The employee operates a loader as part of EE's normal work activity. The employee experienced lower-back pain and reported it to EE's employer on 5-29-2020. The employee did not know if the pain was work related or personal went to medical treatment on EE's own. On 9-21-2020 EE reported to EE's employer that the pain had persisted and the employee was sent to a WC Physician.

Employee was involved in grinding operations and was wearing welding helmet and safety glasses. Employee received medical treatment on 5/25/13 to remove metal sliver from cornea. Employee was given medication and ointment for infection. Employee is fully recovered with no pain or vision impairment as of 5/28/13.

Rock lodged in tail pulley of conveyor. Folded conveyor up to reduce belt tension. Removed rock. While lowering conveyor to operating position, rock fell/rolled from belt. Finger pinched between conveyor frame & rock. Four stitches required to treat injury.
