Employee was putting belt structure together when the top roller fell off the stand pinching left ring finger between roller and rail.

Wet conditions on an incline surface which caused the employee to twist EE's left knee resulting with a torn meniscus.

Employee set a metal skid on an unseen pin that kept the skid off the ground, EE proceeded to torch a drain hole in the skid when the skid teetered off the pin which fell towards EE striking EE's right foot which caused the fracture.

Employee was loosening a bolt when the wrench broke and from that force caused employee to fall forward into the roof bolter hitting chin, this causing the laceration.

EMPLOYEE WAS PULLING A ROCK OFF COAL CHAIN WHEN EE FELT DISCOMFORT IN LOWER ABDOMEM.

Individual was lifting up on the low belt to add a top slide when the individual felt a strain in low back. This resulted in a lost time injury as of 3/20/2023.

EE was transporting supplies when the scoop drove over a rock that was not seen causing the scoop to lift up towards the roof which EE's head came in contact with the roof, jarring EE's neck. The employee worked until 11/22/2022 making it a lost time injury.

DRILL STEEL FELL FROM THE ROOF STRIKING LEFT INDEX FINGER WHICH WAS RESTING ON THE DRILL HEAD. RESULTING IN STITCHES

Employee was moving the section when they slipped on the conveyor belt twisting their right knee which resulted in a meniscus tear.

The individual was in the number 4 entry which was 40" high and 18-18.5' wide in wet and muddy conditions. EE was bending and throwing out bolts on the helper side of the twin boom when EE turned to go back towards the back of the machine and the operator trammed forward and EE's toes got under the cats of the machine resulting in two fractured toes and sutures.

Hit uncharted borehole in MMU 003 section at the face of number four entry.

Employee was moving a detached mobile bridge conveyor when they didn't see a belly in the mine floor which caused the back of the machine to raise towards the roof. The employee was sitting higher than the mushroom post and their head struck the roof causing the roof causing them to jar their neck and resulting in shoulder discomfort and lost time accident.

Employee was driving underground transportation through a set of metal doors when the doors proceeded to swing shut which came into contact with EE's middle finger that was resting on the framework of the transportation this causing the laceration.

Employee got finger pinched between two drill steels. Resulting in stitches.

Employee was shoveling coal from underneath a low seam conveyor belt when EE felt severe pain in the lower back. On 9/29/2021 employee was scheduled for a MRI, the outcome is to be determined.

Employee was lifting a piece of belt structure while working on knees in a low seam coal mine when employee felt pain in groin area. The employee worked until hernia surgery on 8/18/2021.

Employee was putting main line structure together which requires a cable to hold the coal belt off the structure that is when the cable slipped leaving the belt fall striking EE's forearm.

Employee was replacing a head jack cylinder on a miner when the cylinder shifted and struck employee's finger which caused the laceration.

Roof fall above a anchorage point in the belt line of b-mains wall 89. There were two geological features and some water present in this area. The length of the roof fall was approximately 20 foot 6 inches by 65 ft. 21 ft. high above the normal roof.

While helping to move the section, the employee got a finger pinched between the dolly and the scoop bucket as they were moving a cable, which required stitches.

Employee was bolting at the face and went to pry down a loose piece of rock from the roof when another piece of rock fell onto the rock bar as EE slipped and fell into the bar, striking their face/cheek.

Employee was repositioning a roll of conveyor belt to be unrolled in the beltline when ee felt pain in left side middle back area caused by a fractured rib.

Employee was installing a roof bolt in the mine roof when EE lacerated right hand on a roof plate that required stitches.

Employee was setting cribs when ee pinched a finger on left hand between a crib and the mine roof, lacerating that finger that required stitches.

Employee was moving a stack of cribs at the face when the employee pinched left hand between one of the cribs and a mandoor, lacerating a finger on that hand.

Employee was working outside in a maintenance shop cleaning threads with a thread tap when the tap broke causing a laceration on employee's left hand requiring stitches.

Employee was lifting rocks off of the mine floor and putting them on a conveyor belt and soon after felt a pain and numbness in the shoulder/arm. The belt line is typically around 42" high. EE recently sought medical treatment and was diagnosed with a disc herniation and went off work on 12/28/2018 pending surgery.

Employee was changing a hydraulic cylinder on a Fletcher twin boom bolter when the cylinder fell, pinching hand between the cylinder and the mine floor. This caused a laceration on the palm of right hand.

Employee was removing rocks from on top of a Joy 14-10 miner when ee caught -left hand between 2 rocks and lacerated a finger on left hand.

While the employee was operating a Fletcher twin boom bolter, EE pushed on the chuck and the drill steel fell and lacerated EE's left elbow.

employee was working at the face when they slipped and fell. EE tried to break the fall with the right hand and lacerated the fingers on that hand on some sharp rocks.

Employee was lifting structure at the face when ee felt pain in lower right abdomen.

Roof fall in the # 9 room of the C butt that measured 20 ft. wide x 70 ft. long above anchorage. A visible geological feature was identified in the adjacent entries contributing to this fall.

While lifting miner cables, EE felt a pain in the left testicle.

Adverse roof conditions located near stream bed.

While removing rock from canopy, finger got pinched between rock and canopy.

Miner was lifting belt structure when ee felt pain in lower back on 8/4/2017, ee continued to work until 10/24/2017 when ee had to seek treatment and miss work, making it a reportable accident.

Miner developed pain in left knee on 7/12/17 caused by a repetitive crawling motion. EE continued to work until 8/21/17 when ee had to seek treatment and miss work, making it a reportable accident.

Miner was operating a twin boom bolter installing roof bolts at the face when a rock 30"x15"x3" fell from the roof onto EE's right ankle.

Carpal tunnel from repeated trauma from every day work

Unplanned roof fall in the C-2 butt, room #2, 1 crosscut outby spad #608. At the time of the fall, the primary roof support was 4 ft. Fully grouted resin bolts.

Employee was bending bolts by the kitchen of the twin boom when a piece of rock 22" x 25" x 9" fell onto EE's lower leg.

Employee strained lower back while loading conveyor structure into a scoop bucket.

Unplanned roof fall in the B-Mains at 1 crosscut Inby spad #434. Fall occurred right above anchorage point, which was bolted with 6 FT. fully grouted resin bolts. At the time of the fall, due to being a weekend, the mine was idle.

Employee was just walking when slipped on a slippery spot and fell into a shallow hole.

Employee hit self in the mouth with a drill steel as employee was removing it from the hole.

Employee tripped and hit his lip on the scoop he was going to operate. Not serious, no lost time.
