EE WALKED UP TO A CONVEYOR TO MAKE AN ADJUSTMENTAND A STONE CAME OFF AND HIT HIM IN THE HEAD. HERECEIVED A CUT ABOVE HIS LEFT EYE, WHICH REQUIRED 10 STITCHES. EE WAS WEARING HIS HARD HAT AND S AFETY GLASSES AT THE TIME OF THE INCIDENT.

E E SUSTAINED A RIGHT INDEX FINGER--BROKEN AND 25 STITCHES TO HAND. HE RETURNED TO WORK NEXT DAY--NO WORK TIME LOST.

EE WAS CLIMBING UP LOADER LADDER AND SLIPPED OFF A STEP AND TWISTED HIS LEFT ANKLE. HE WENT TO DR AND WAS GIVEN SLIP TO BE OFF WORK UNTIL 3-14-95, WHEN HE WOULD BE REEXAMINED. DR REEXAMINED HI M ON THIS DATE AND FOUND THAT HE HAD A BROKEN FIBULA AND WOULD BE OFF WORK APPROX 6 WEEKS.

AT END OF DAY, 3-23-94, HE WENT TO C LINIC, WHERE THEY REMOVED FOREIGN PARTICLE FROM HIS EYE. HE WAS RELEASED TO RETURN TO WORK WITHOUT RESTRICTIONS.
