EE WAS CLIMBING ON CONVEYOR & GOT HIS LEG CAUGHTIN FRAMEWORK. HE FELL & LANDED ON HIS NECK & SHOULDER, ALSO GOT CUTS ON EACH OF HIS LEGS [MINOR]. HE WENT TO DOCTOR 12/14/94 & DOCTOR TOLD HIM T O TAKE 3 DAYS OFF.

HE WAS STANDING ON TOP OF 120' STACKER CONVEYOR. THEY WERE FOLDING IT UP GETTING READY TO MOVE TO ANOTHER PIT. THE CHAIN BROKE, WHICH WAS HOOKED TO A LOADER AND THE CONVEYOR AND DROPPED THE CO NVEYOR. HE FELL DOWN AND JARRED HIS BACK. HE WAS SENT TO THE LAKELAND HOSPITAL AND THEY TOOK X-RAYS. SAID HIS BACK WAS STRAINED AND TO TAKE 2 TO 3 DAYS OFF.
EE WAS MOUNTING AN ELECTRIC MOTOR WELDER ONTO SIDE OF CONVEYOR HE THOUGHT HE HAD BOLTS RIGHT AND LET GO OF THE MOTOR. IT WAS NOT TIGHT AND FELL ON HIS RIGHT HAND LACERATING HIS RIGHT RING FING ER RECEIVED 5 STITCHES AND ALSO ALOS HAIRLINE FRACTURE OF THAT FINGER.
