Employee fell off ladder and fell on left shoulder. Mild left shoulder separation and left chest contusion. Employee is working on restrictions til 7/30/07 when he sees his doctor again.

HE WAS STANDING ON TOP OF 120' STACKER CONVEYOR. THEY WERE FOLDING IT UP GETTING READY TO MOVE TO ANOTHER PIT. THE CHAIN BROKE, WHICH WAS HOOKED TO A LOADER AND THE CONVEYOR AND DROPPED THE CO NVEYOR. HE FELL DOWN AND JARRED HIS BACK. HE WAS SENT TO THE LAKELAND HOSPITAL AND THEY TOOK X-RAYS. SAID HIS BACK WAS STRAINED AND TO TAKE 2 TO 3 DAYS OFF.
