Employee was changing shaker springs on Shaker C in Plant. While installing the new spring, the shaker deck dropped and pinched left middle finger resulting a laceration that required 13 stitches to close wound.

Employee was opening a knife valve with a pry bar. The pry bar slipped and the employee fell backwards and hit his head on a hopper causing a laceration to the back of the head.

On 8/28/2012 employee was shoveling gypsum, which weighs approximately 15 lbs per shovel into a loader buckets. During this time, the employee seemed to pull/sprain a muscle or tendon in his elbow region. It seems to be a very minor injury. Employee did return to work during the ten day, however, went back on disablity.

Employee was performing routine maintenance in the Sand Plant. He reached over a piece of pipe and was stung by a spider. Three days after spider sting, employee needed to go to doctor for treatment becuase the bite area had become infected. Wound was lanced and employee treated with antibiotics.
