EMPLOYEE STATED THAT THE SEAT IN HIS LOADER WAS GIVING HIM LOWER BACK PAIN. NOTIFIED THIS OFFICE ON 3/14/01 OF HIS PROBLEM. LAST DAY OF WORK WAS 3/5/01.

HE FINISHED HIS SHIFT THAT DAY AND ALSO WORKED THE NEXT DYA BEFORE GOING TO A DOCTOR ON THE EVENING OF 8-30-91.
