When ee and coworker were removing the tail pulley guard he stepped over uneven ground and lost his footing. When he stumbled he caught himself not completely falling and hurt his back. Employee has worked full duty with some back discomfort until 3/09/2012 when he had surgery to alleviate the pain. - This surgery makes the incident reportable lost time 03/12/2012.

A Fed-X driver delivered a package while walking to his truck he slipped on the ice falling backwards hitting his head on the asphalt. Lost consciousness for about 1 min before regaining consciousness. Was transported to the hsp by ambulance and was treated for a concussion and cut to the back of the head. He received 6 stitches and was released

While cleaning around #2 conveyor Injury moved the corner post to the guard. He was shaking the post to remove loose material when he felt pain in his knee. He continued working until 12pm when he said the pain had gotten worse, he reported incident to supervisor. *Employee worked restricted duty for three days before returning to regular duty.

WHILE LIFTING A PIECE OF WIRE CLOTH ONTO THE WASHING SCREEN, EMPLOYEE HEARD A "POP" IN HIS LEFT ARM. HIS CO-WORKER GOT HIM OFF THE SCREEN TOWER WITH A MANLIFT. HE WAS THEN TAKEN TO THE LOCAL HOSPITAL WERE HE WAS DIAGNOSED WITH A TORN BICEP TENDON. X-RAYS CONFIRMED NO BONE DAMAGE. HE WAS INSTRUCTED TO SEE AN ORTHOPEDIC SPECIALIST TO FIND OUT HOW EXTENSIVE THE DAMAGE WAS.

EE AND SEVERAL OTHERS WERE CHANGING THE TEETH AND WEAR CORNER SHROUDS ON OUR PIT LOADER. THEY WERE KNOCKING THE SHROUDS OFF WITH A SLEDGE HAMMER. AFTER THE JOB WAS COMPLETED, EE STARTED WALKIN G OVER TO TRUCK TO LEAVE FOR HOME WHEN HE FELT SOMETHING WET IN HIS WORK BOOT. HE STOPPED AND REMOVED HIS LEFT BOOT AND DISCOVERED HIS FOOT WAS BLEEDING. HE TOLD HIS FELLOW EE HE FELT SOMETHI

THAT EMPLOYEE HAD A #2 SPRAIN OF HIS RIGHT ANKLE. EMPLOYEE RETURNED TO WORK. ON 10-3-97 EMPLOYEE HAD SURGERY ON THAT RIGHT ANKLE TO REMOVE A BONE SPUR.

AS EMPLOYEE AND A CONTRACTOR WERE REMOVING A GUARD FOR REPAIR, THE GUARD, AFTER BEING CUT LOOSE, SLIPPED OUT OF EMPLOYEE'S HANDS. WHEN HE WENT TO GRAB IT TO STOP IT FROM FALLING, THE EMPLOYEE' S HAND GOT TRAPPED BETWEEN THE GUARD AND THE HANDRAIL.

HE GABBED A HOLD OF HTE PUMP WITH A FREE HAND AND HELD ON TO THE BUCKET WITH THE OTHER HAND. EMPLOYEE DIDN'T HIT THE GROUND BUT STRAINED AND PULLED HIS LOWER BACK MUSCLES PREVENTING HIS FALL.
