Miner alleged he was climbing ladder on floating conveyor. Ladder measured about 5.5feet high. He allegedly lost footing and fell. Upon investigation miner does not remember any more information and this incident was not witnessed. Diagnosis was a concussion and lumbar contusion.

Employee was holding a spud bar while another employee was striking the bar with a sledge hammer to break a rock from crusher. The hammer missed the spud bar and struck the employee's left wrist.

OPERATOR WAS STRIPPING A WOODED AREA.WHILE PUSHING A FELLED TREE A LIMB SWUNG BACK AND STRUCK HOM ON THE TIP OF HIS BABY FINGER.THE FINGER NAIL BROKE LOOSE.UPON EXAMINATION BY DOCTOR THE NAIL WAS CUT BACK,TWO STTITCHES APPLIED AND THEN A SPLINT WAS APPLIED FOR THE CRACKED BONE.OPERATOR RETURNED TO WORK.
